O que é?

SAS é um dos defeitos congénitos mais frequentes no cão (raro no gato). Tecido fibromuscular forma um anel sob a válvula aórtica e obstrui o LVOT. O ventrículo esquerdo bombeia contra a estreiteza: sobrecarga de pressão e hipertrofia concêntrica. A parede espessa torna-se isquémica com risco de arritmias ou insuficiência.

"The primary lesion of SAS is the abnormal presence of endocardial tissue upstream of the aortic valve causing obstruction of the LVOT [10]. Sometimes the anterior (septal) mitral leaflet may be involved."

Causas e fatores de risco

Congénito; o estreitamento costuma progredir no primeiro ano. Forte base genética: Newfoundland autossómico dominante. Também: Golden Retriever, Rottweiler, Boxer, German Shepherd Dog, Great Dane, German Shorthaired Pointer, Bouvier des Flandres, Samoyed, Dogue de Bordeaux, Bull Terrier, Greyhound.

Sinais

Ligeira muitas vezes silenciosa. Cardinal: sopro de ejeção sistólico áspero à esquerda. Frequentes: intolerância ao exercício, fadiga, síncope, pulsos femorais fracos/atrasados, thrill precordial, dispneia, taquiarritmias ventriculares, morte súbita. Ocasionais: edema pulmonar, sopro diastólico de insuficiência aórtica.

Diagnóstico

Auscultação, pulsos, thrill. Eco transtorácico (padrão-ouro) e Doppler (velocidade = gravidade). Eco transesofágico, radiografias, ECG (arritmias), raramente cateter.

Tratamento

Nenhum fármaco dissolve o anel. Betabloqueadores, restrição estrita de exercício se moderada–grave. Valvuloplastia com balão e ressecção aberta em centros especializados, sucesso a longo prazo muitas vezes limitado ou alto risco.

Prognóstico

Ligeira: esperança muitas vezes normal. Moderada–grave: reservado a mau (ICC, morte súbita). Maior risco de endocardite aórtica:

"Animals with SAS are thought to be at higher risk for aortic valve endocarditis because of jet lesion injury to the underside of the valve..."

"Heart failure results when ventricular diastolic and atrial pressures are elevated. Cardiac arrhythmias can contribute to the onset of CHF. Furthermore, the combination of outflow obstruction, paroxysmal arrhythmias, and/or low-output signs can result from severe outflow obstruction, tachyarrhythmia."

Prevenção

Criação: eco antes de reproduzir; excluir afectados mesmo ligeiros. Velocidades equívocas só com sinais associados.

"Peak velocities in the equivocal range between these values may indicate the presence of mild SAS, especially if there is other evidence of disease such as a subaortic ridge, disturbed flow in the outflow tract or ascending aorta with an abrupt increase in velocity, and aortic regurgitation. This is mainly of concern when selecting animals for breeding."
