TL;DR. La stenosi sottoaortica è un difetto cardiaco congenito: un anello di tessuto anomalo sotto la valvola aortica restringe l'efflusso sinistro, sovraccarica il miocardio e può causare sincope, intolleranza all'esercizio o morte improvvisa.

Cos'è

SAS è uno dei difetti congeniti più comuni del cane (raro nel gatto). Tessuto fibromuscolare forma un anello sotto la valvola aortica e ostruisce il LVOT. Il ventricolo sinistro pompa contro lo stretto: sovraccarico di pressione e ipertrofia concentrica. La parete spessa diventa ischemica con rischio di aritmie o scompenso.

"The primary lesion of SAS is the abnormal presence of endocardial tissue upstream of the aortic valve causing obstruction of the LVOT [10]. Sometimes the anterior (septal) mitral leaflet may be involved."

Cause e fattori di rischio

Congenito; lo stretto spesso peggiora nel primo anno. Forte base genetica: Newfoundland autosomico dominante. Anche: Golden Retriever, Rottweiler, Boxer, German Shepherd Dog, Great Dane, German Shorthaired Pointer, Bouvier des Flandres, Samoyed, Dogue de Bordeaux, Bull Terrier, Greyhound.

Segni

Lieve spesso silente. Cardinale: soffio di eiezione sistolico aspro a sinistra. Comuni: intolleranza all'esercizio, fatica, sincope, polsi femorali deboli/ritardati, thrill precordiale, dispnea, tachiaritmie ventricolari, morte improvvisa. Occasionali: edema polmonare, soffio diastolico da insufficienza aortica.

Diagnosi

Auscultazione, polsi, thrill. Eco transtoracico (gold standard) e Doppler (velocità = gravità). Eco transesofageo, radiografie, ECG (aritmie), raramente catetere.

Trattamento

Nessun farmaco dissolve l'anello. Beta-bloccanti, restrizione rigorosa dell'esercizio se moderato–grave. Valvuloplastica con palloncino e resezione aperta in centri specialistici, successo a lungo termine spesso limitato o alto rischio.

Prognosi

Lieve: aspettativa spesso normale. Moderato–grave: riservata a scarsa (scompenso, morte improvvisa). Maggior rischio di endocardite aortica:

"Animals with SAS are thought to be at higher risk for aortic valve endocarditis because of jet lesion injury to the underside of the valve..."

"Heart failure results when ventricular diastolic and atrial pressures are elevated. Cardiac arrhythmias can contribute to the onset of CHF. Furthermore, the combination of outflow obstruction, paroxysmal arrhythmias, and/or low-output signs can result from severe outflow obstruction, tachyarrhythmia."

Prevenzione

Riproduzione: eco prima dell'accoppiamento; escludere i soggetti affetti anche lievi. Velocità equivoche solo con segni associati.

"Peak velocities in the equivocal range between these values may indicate the presence of mild SAS, especially if there is other evidence of disease such as a subaortic ridge, disturbed flow in the outflow tract or ascending aorta with an abrupt increase in velocity, and aortic regurgitation. This is mainly of concern when selecting animals for breeding."
