TL;DR. La sténose sous-aortique est une malformation cardiaque congénitale: un anneau de tissu anormal sous la valve aortique restreint l'éjection gauche, surcharge le myocarde et peut entraîner syncope, intolérance à l'effort ou mort subite.

Un anneau fibreux sous la valve aortique rétrécit la voie d'éjection gauche.

Qu'est-ce que c'est?

SAS est l'une des malformations congénitales les plus fréquentes du chien (rare chez le chat). Un tissu fibro-musculaire forme un anneau sous la valve aortique et obstrue le LVOT. Le ventricule gauche pompe contre l'obstacle: surcharge de pression et hypertrophie concentrique. La paroi épaisse devient ischémique avec risque d'arythmies ou d'insuffisance.

"The primary lesion of SAS is the abnormal presence of endocardial tissue upstream of the aortic valve causing obstruction of the LVOT [10]. Sometimes the anterior (septal) mitral leaflet may be involved."

Causes et facteurs de risque

Congénital; le rétrécissement s'aggrave souvent la première année. Forte composante génétique: Newfoundland dominant autosomique. Aussi: Golden Retriever, Rottweiler, Boxer, German Shepherd Dog, Great Dane, German Shorthaired Pointer, Bouvier des Flandres, Samoyed, Dogue de Bordeaux, Bull Terrier, Greyhound.

Signes

Formes légères souvent silencieuses. Cardinal: souffle d'éjection systolique rude à gauche. Fréquents: intolérance à l'effort, fatigue, syncope, pouls fémoraux faibles/retardés, thrill précordial, dyspnée, tachyarythmies ventriculaires, mort subite. Occasionnels: œdème pulmonaire, souffle diastolique d'insuffisance aortique.

Diagnostic

Auscultation, pouls, thrill. Écho transthoracique (référence) et Doppler (vitesse = sévérité). Écho transœsophagien, radiographies, ECG (arythmies), rarement cathéter.

Traitement

Aucun médicament ne dissout l'anneau. Bêtabloquants, restriction d'effort stricte si modéré–sévère. Valvuloplastie au ballon et résection ouverte en centres spécialisés, succès long terme souvent limité ou risque élevé.

Pronostic

Léger: espérance souvent normale. Modéré–sévère: réservé à mauvais (CHF, mort subite). Risque accru d'endocardite aortique:

"Animals with SAS are thought to be at higher risk for aortic valve endocarditis because of jet lesion injury to the underside of the valve..."

"Heart failure results when ventricular diastolic and atrial pressures are elevated. Cardiac arrhythmias can contribute to the onset of CHF. Furthermore, the combination of outflow obstruction, paroxysmal arrhythmias, and/or low-output signs can result from severe outflow obstruction, tachyarrhythmia."

Prévention

Élevage: écho avant reproduction; exclure les atteints même légers. Vitesses équivoques avec signes associés seulement.

"Peak velocities in the equivocal range between these values may indicate the presence of mild SAS, especially if there is other evidence of disease such as a subaortic ridge, disturbed flow in the outflow tract or ascending aorta with an abrupt increase in velocity, and aortic regurgitation. This is mainly of concern when selecting animals for breeding."

Quand appeler le vétérinaire

Urgence: syncope/collapsus, dyspnée/toux, faiblesse extrême, muqueuses pâles ou bleues.

Pour certaines races

Terre-Neuve: dépistage strict, exclure même les formes légères/équivoques. Boxer et Golden Retriever: écho précis par un cardiologue.
