Subaortic stenosis (SAS) is a common congenital heart defect in dogs, and occasionally cats, characterized by a narrowing just below the aortic valve. Wat is het?
SAS is een van de meest voorkomende aangeboren afwijkingen bij de hond (zeldzaam bij de kat). Fibromusculair weefsel vormt een ring onder de aortaklep en belemmert de LVOT. De linkerkamer pompt tegen de vernauwing: drukoverbelasting en concentrische hypertrofie. De dikke wand wordt ischemiegevoelig met risico op aritmieën of falen.
"The primary lesion of SAS is the abnormal presence of endocardial tissue upstream of the aortic valve causing obstruction of the LVOT [10]. Sometimes the anterior (septal) mitral leaflet may be involved."
Oorzaken en risicofactoren
Congenitaal; de vernauwing verergert vaak in het eerste jaar. Sterk genetisch: Newfoundland autosomiaal dominant. Ook: Golden Retriever, Rottweiler, Boxer, German Shepherd Dog, Great Dane, German Shorthaired Pointer, Bouvier des Flandres, Samoyed, Dogue de Bordeaux, Bull Terrier, Greyhound.
Signalen
Mild vaak stil. Kardinaal: ruw systolisch uitdrijvingsgeruis links. Vaak: inspanningsintolerantie, vermoeidheid, syncope, zwakke/vertraagde femorale pulsen, precordiale thrill, dyspneu, ventriculaire tachyaritmieën, plotselinge dood. Soms longoedeem, diastolisch geruis bij aortainsufficiëntie.

Inspanningsintolerantie en vermoeidheid komen vaak voor bij matige tot ernstige SAS.
Diagnostiek
Auscultatie, pulsen, thrill. Transthoracaal echo (gouden standaard) en Doppler (snelheid = ernst). Transoesofageaal echo, röntgen, ECG (aritmieën), zelden katheter.
Behandeling
Geen medicijn lost de ring op. Bètablokkers, strikte inspanningsbeperking bij matig–ernstig. Ballonvalvuloplastie en open resectie in specialistische centra, langetermijnsucces vaak beperkt of hoog risico.
Prognose
Mild: vaak normale levensverwachting. Matig–ernstig: gereserveerd tot slecht (CHF, plotse dood). Hoger risico op aortaklep-endocarditis:
"Animals with SAS are thought to be at higher risk for aortic valve endocarditis because of jet lesion injury to the underside of the valve..."
"Heart failure results when ventricular diastolic and atrial pressures are elevated. Cardiac arrhythmias can contribute to the onset of CHF. Furthermore, the combination of outflow obstruction, paroxysmal arrhythmias, and/or low-output signs can result from severe outflow obstruction, tachyarrhythmia."
Preventie
Fok: echo vóór dekking; uitsluiten van aangedane (ook milde). Equivocale snelheden alleen met bijkomende tekenen.
"Peak velocities in the equivocal range between these values may indicate the presence of mild SAS, especially if there is other evidence of disease such as a subaortic ridge, disturbed flow in the outflow tract or ascending aorta with an abrupt increase in velocity, and aortic regurgitation. This is mainly of concern when selecting animals for breeding."

Transthoracaal echo is het referentie-instrument om SAS te identificeren.
Wanneer de dierenarts bellen
Spoed: syncope/collaps, dyspneu/hoesten, extreme zwakte, bleke/blauwe slijmvliezen.
Voor specifieke rassen
Newfoundlander: strikte screening, ook milde/equivocale uitsluiten. Boxer en Golden Retriever: precies echo door cardioloog.
