Hvad er det?
SAS er en af de mest almindelige medfødte defekter hos hund (sjælden hos kat). Fibromuskulært væv danner en ring under aortaklappen og blokerer LVOT. Venstre ventrikel pumper mod forsnævringen: trykoverbelastning og koncentrisk hypertrofi. Den tykke væg bliver iskemisk med risiko for arytmier eller svigt.
"The primary lesion of SAS is the abnormal presence of endocardial tissue upstream of the aortic valve causing obstruction of the LVOT [10]. Sometimes the anterior (septal) mitral leaflet may be involved."
Årsager og risikofaktorer
Medfødt; forsnævringen forværres ofte det første år. Stærk genetik: Newfoundland autosomal dominant. Også: Golden Retriever, Rottweiler, Boxer, German Shepherd Dog, Great Dane, German Shorthaired Pointer, Bouvier des Flandres, Samoyed, Dogue de Bordeaux, Bull Terrier, Greyhound.
Tegn
Mild ofte stille. Kardinal: ru systolisk uddrivningsmislyd venstre. Almindeligt: motionsintolerance, træthed, synkope, svage/forsinkede femoralpulser, precordial thrill, dyspnø, ventrikulære takyarytmier, pludselig død. Af og til lungeødem, diastolisk mislyd ved aortainsufficiens.

Motionsintolerance og træthed er almindelige ved moderat til svær SAS.
Diagnostik
Auskultation, pulser, thrill. Transthorakalt ekko (guldstandard) og Doppler (hastighed = sværhedsgrad). Transøsofagealt ekko, røntgen, EKG (arytmier), sjældent kateter.
Behandling
Ingen medicin opløser ringen. Betablokkere, streng motionsbegrænsning ved moderat–svær. Ballonvalvuloplastik og åben resektion på specialcenter, langsigtet succes ofte begrænset eller høj risiko.
Prognose
Mild: ofte normal levetid. Moderat–svær: reserveret til dårlig (CHF, pludselig død). Højere risiko for aortaklap-endokarditis:
"Animals with SAS are thought to be at higher risk for aortic valve endocarditis because of jet lesion injury to the underside of the valve..."
"Heart failure results when ventricular diastolic and atrial pressures are elevated. Cardiac arrhythmias can contribute to the onset of CHF. Furthermore, the combination of outflow obstruction, paroxysmal arrhythmias, and/or low-output signs can result from severe outflow obstruction, tachyarrhythmia."
Forebyggelse
Avl: ekko før avl; udeluk afficerede også milde. Tvetydige hastigheder kun med tillægsfund.
"Peak velocities in the equivocal range between these values may indicate the presence of mild SAS, especially if there is other evidence of disease such as a subaortic ridge, disturbed flow in the outflow tract or ascending aorta with an abrupt increase in velocity, and aortic regurgitation. This is mainly of concern when selecting animals for breeding."
