Hva er det?

SAS er en av de vanligste medfødte defektene hos hund (sjelden hos katt). Fibromuskulært vev danner en ring under aortaklaffen og blokkerer LVOT. Venstre ventrikkel pumper mot forsnevringen: trykkoverbelastning og konsentrisk hypertrofi. Den tykke veggen blir iskemisk med risiko for arytmier eller svikt.

"The primary lesion of SAS is the abnormal presence of endocardial tissue upstream of the aortic valve causing obstruction of the LVOT [10]. Sometimes the anterior (septal) mitral leaflet may be involved."

Årsaker og risikofaktorer

Medfødt; forsnevringen forverres ofte første år. Sterk genetikk: Newfoundland autosomal dominant. Også: Golden Retriever, Rottweiler, Boxer, German Shepherd Dog, Great Dane, German Shorthaired Pointer, Bouvier des Flandres, Samoyed, Dogue de Bordeaux, Bull Terrier, Greyhound.

Tegn

Mild ofte stille. Kardinal: ru systolisk utdrivningsbilyd venstre. Vanlig: anstrengelsesintoleranse, tretthet, synkope, svake/forsinkede femoralpulser, precordial thrill, dyspné, ventrikulære takyarytmier, plutselig død. Av og til lungeødem, diastolisk bilyd ved aortainsuffisiens.

Anstrengelsesintoleranse og tretthet er vanlige ved moderat til alvorlig SAS.

Diagnostikk

Auskultasjon, pulser, thrill. Transthorakalt ekko (gullstandard) og Doppler (hastighet = alvorlighet). Transøsofagealt ekko, røntgen, EKG (arytmier), sjelden kateter.

Behandling

Ingen medisin løser ringen. Betablokkere, streng anstrengelsesbegrensning ved moderat–alvorlig. Ballongvalvuloplastikk og åpen reseksjon på spesialsenter, langsiktig suksess ofte begrenset eller høy risiko.

Prognose

Mild: ofte normal levetid. Moderat–alvorlig: reservert til dårlig (CHF, plutselig død). Høyere risiko for aortaklaff-endokarditt:

"Animals with SAS are thought to be at higher risk for aortic valve endocarditis because of jet lesion injury to the underside of the valve..."

"Heart failure results when ventricular diastolic and atrial pressures are elevated. Cardiac arrhythmias can contribute to the onset of CHF. Furthermore, the combination of outflow obstruction, paroxysmal arrhythmias, and/or low-output signs can result from severe outflow obstruction, tachyarrhythmia."

Forebygging

Avl: ekko før avl; utelukk affiserte også milde. Tvetydige hastigheter bare med tilleggsfunn.

"Peak velocities in the equivocal range between these values may indicate the presence of mild SAS, especially if there is other evidence of disease such as a subaortic ridge, disturbed flow in the outflow tract or ascending aorta with an abrupt increase in velocity, and aortic regurgitation. This is mainly of concern when selecting animals for breeding."

Akutt: synkope/kollaps, dyspné/hoste, ekstrem svakhet, bleke/blå slimhinner.

Newfoundland: streng screening, utelukk også mild/tvetydig. Boxer og golden retriever: presist ekko av kardiolog.
