TL;DR. A IMPA é uma inflamação tratável de várias articulações em que o sistema imune ataca a sinóvia; febre, dor e marcha «em ovos» são típicas.

Postura rígida e relutância em se mover são sinais precoces frequentes de dor articular na IMPA.

O que é?

A IMPA é uma inflamação não séptica de várias articulações por deposição de imunocomplexos na sinóvia. Geralmente não erosiva (osso intacto). Primária (idiopática) ou reativa (secundária a infecção, inflamação, tumor, fármacos, vacinação) ou parte de LES.

Causas e fatores de risco

IMPA idiopática, LES, poliartrite reativa, síndromes de raça. Maior risco: Weimaraner, Newfoundland, Chinese Shar-Pei, Boxer, Bernese Mountain Dog, German Shorthaired Pointer, Beagle, Akita/Akita Inu.

Sinais a observar

  • Febre (cardinal): muitas vezes sem resposta a antibióticos.
  • Claudicação e marcha «em ovos» (frequentes)
  • Dor, rigidez, edema articular (frequentes)
  • Letargia, anorexia (frequentes)
  • Dor cervical (ocasional)

Como os veterinários diagnosticam

  • Análise sinovial [padrão-ouro]: várias articulações; WBC/neutrófilos elevados, cultura negativa.
  • Radiografias: edema de tecidos moles, sem erosões na IMPA não erosiva.
  • Hemograma; ANA se suspeita de LES.

"Synovial fluid analysis reveals an increase in both white blood cell (WBC) count and the percentage of neutrophils in affected joints, but synovial fluid culture is negative. Even if the underlying inflammatory disease is infectious, the polyarthritis in these patients is caused by synovial deposition of circulating immune complexes, not by infection of the joints."

Opções de tratamento

"Additional immunosuppressive agents are rarely necessary, because idiopathic nonerosive IMPA is easy to control in most patients. If the polyarthritis is refractory to treatment, the patient should be reevaluated for infectious disease, reactive polyarthritis, and erosive disease prior to considering alternative immunosuppressive agents."

Prognóstico

Bom na IMPA idiopática não erosiva; alguns pacientes precisam de dose baixa a longo prazo. Pior em LES e certos síndromes de raça.

Prevenção

Forma idiopática não evitável. Reativa: tratar cedo as infecções e discutir o plano vacinal.

Quando chamar o veterinário

Febre, rigidez, letargia: consulta. Urgência se febre muito alta, incapacidade de se manter em pé, dor cervical intensa, colapso ou dispneia sob imunossupressores.

Para raças específicas

Vigiar rigidez e febre em raças predispostas; os akita podem ter síndrome grave com meningite e pior prognóstico.
