¿Qué es?

La IMPA es una inflamación no séptica de varias articulaciones por depósito de inmunocomplejos en la sinovial. Suele ser no erosiva (hueso intacto). Primaria (idiopática) o reactiva (secundaria a infección, inflamación, tumor, fármacos, vacunación) o parte de un LES.

Causas y factores de riesgo

IMPA idiopática, LES, poliartritis reactiva, síndromes de raza. Mayor riesgo: Weimaraner, Newfoundland, Chinese Shar-Pei, Boxer, Bernese Mountain Dog, German Shorthaired Pointer, Beagle, Akita/Akita Inu.

Signos a vigilar

  • Fiebre (cardinal): a menudo no responde a antibióticos.
  • Cojera y marcha «sobre huevos» (frecuentes)
  • Dolor, rigidez, hinchazón articular (frecuentes)
  • Letargo, anorexia (frecuentes)
  • Dolor de cuello (ocasional)

Cómo lo diagnostican los veterinarios

  • Análisis de sinovial [estándar de oro]: varias articulaciones; WBC/neutrófilos altos, cultivo negativo.
  • Radiografías: edema de tejidos blandos, sin erosiones en IMPA no erosiva.
  • Hemograma; ANA si se sospecha LES.

"Synovial fluid analysis reveals an increase in both white blood cell (WBC) count and the percentage of neutrophils in affected joints, but synovial fluid culture is negative. Even if the underlying inflammatory disease is infectious, the polyarthritis in these patients is caused by synovial deposition of circulating immune complexes, not by infection of the joints."

Opciones de tratamiento

"Additional immunosuppressive agents are rarely necessary, because idiopathic nonerosive IMPA is easy to control in most patients. If the polyarthritis is refractory to treatment, the patient should be reevaluated for infectious disease, reactive polyarthritis, and erosive disease prior to considering alternative immunosuppressive agents."

Pronóstico

Bueno en IMPA idiopática no erosiva; algunos pacientes necesitan dosis baja a largo plazo. Peor en LES y ciertos síndromes de raza.

Prevención

La forma idiopática no se puede prevenir. Reactiva: tratar pronto infecciones y comentar el plan vacunal.

Cuándo llamar al veterinario

Fiebre, rigidez, letargo: cita. Urgencia si fiebre muy alta, incapacidad para mantenerse en pie, dolor cervical intenso, colapso o disnea con inmunosupresores.

Para razas concretas

Vigilar rigidez y fiebre en razas predispuestas; los akita pueden tener un síndrome grave con meningitis y peor pronóstico.
