Steife Haltung und Bewegungsunlust sind häufige frühe Zeichen von Gelenkschmerzen bei IMPA.

Was ist das?

IMPA ist eine nicht septische Entzündung mehrerer Gelenke durch Ablagerung von Immunkomplexen in der Synovia. Meist nonerosiv (Knochen intakt). Primär (idiopathisch) oder reaktiv (sekundär zu Infektion, Entzündung, Tumor, Medikamenten, Impfung) oder Teil von SLE.

Ursachen und Risikofaktoren

Idiopathische IMPA, SLE, reaktive Polyarthritis, rassespezifische Syndrome. Höheres Risiko: Weimaraner, Newfoundland, Chinese Shar-Pei, Boxer, Bernese Mountain Dog, German Shorthaired Pointer, Beagle, Akita/Akita Inu.

Anzeichen

  • Fieber (kardinal): oft antibiotikaresistent.
  • Lahmheit und „Eierlauf“-Gang (häufig)
  • Schmerz, Steifheit, Gelenkschwellung (häufig)
  • Lethargie, Anorexie (häufig)
  • Nackenschmerzen (gelegentlich)

Diagnose

  • Synoviaanalyse [Goldstandard]: mehrere Gelenke; hohe WBC/Neutrophile, Kultur negativ.
  • Röntgen: Weichteilschwellung, keine Erosionen bei nonerosiver IMPA.
  • CBC; ANA bei SLE-Verdacht.

"Synovial fluid analysis reveals an increase in both white blood cell (WBC) count and the percentage of neutrophils in affected joints, but synovial fluid culture is negative. Even if the underlying inflammatory disease is infectious, the polyarthritis in these patients is caused by synovial deposition of circulating immune complexes, not by infection of the joints."

Behandlung

"Additional immunosuppressive agents are rarely necessary, because idiopathic nonerosive IMPA is easy to control in most patients. If the polyarthritis is refractory to treatment, the patient should be reevaluated for infectious disease, reactive polyarthritis, and erosive disease prior to considering alternative immunosuppressive agents."

Prognose

Bei idiopathischer nonerosiver IMPA meist gut; manchen Patienten brauchen langfristig niedrige Dosen. Schlechter bei SLE und manchen Rassesyndromen.

Vorbeugung

Idiopathische Form nicht verhinderbar. Reaktiv: Infektionen früh behandeln und Impfplan besprechen.

Wann den Tierarzt rufen

Fieber, Steifheit, Lethargie: Termin. Notfall bei sehr hohem Fieber, Unfähigkeit zu stehen, starken Nackenschmerzen, Kollaps oder Atemnot unter Immunsuppression.

Bei bestimmten Rassen

Bei prädisponierten Rassen auf Steifheit und Fieber achten; Akitas können ein schweres Syndrom mit Meningitis und schlechterer Prognose haben.
