Wat is het?

Feochromocytoom is een zeldzame tumor van de bijnieren (kleine, kapvormige organen voor de nieren). Het ontstaat uit chromaffiene cellen van het bijniermerg, die «vecht-of-vlucht»-hormonen (catecholamines: adrenaline en noradrenaline) maken.

Normaal in minuscule, gecontroleerde hoeveelheden. Bij feochromocytoom slaan tumorcellen massale, onvoorspelbare catecholamine-golven op en scheiden die uit: ernstige vaatvernauwing, snelle hartslag en levensbedreigende hypertensie.

Verwante extra-adrenale tumoren (paragangliomen) zijn uitzonderlijk bij hond en kat.

Omdat deze hormonen bijna elk orgaansysteem raken, is het beeld vaak polymorf en lastig te managen.

Oorzaken en risicofactoren

Oorzaak onbekend; meestal als spontaan beschouwd. Vooral oudere dieren. Geen gedocumenteerde raspredispositie in standaard klinische bronnen. Langzaam groeiend maar lokaal invasief; vaak lang aanwezig voor detectie.

Signalen om op te letten

Vaak episodisch («aanvallen») bij hormoonafgifte; tussen episodes kan het dier normaal lijken.

Meerfasig; symptomen overlappen met frequentere endocrinopathieën (Cushing).

Behandelopties

Twee fasen: medische stabilisatie, daarna chirurgie.

"A period of medical therapy intended to reverse the effects of excessive adrenergic stimulation, followed by surgical removal of the tumor, is the treatment of choice for pheochromocytoma.", Internal Medicine 5th p.891

Fase 1

Fase 2: adrenalectomie

Complexe high-risk chirurgie (idealiter specialist). Manipulatie kan massale catecholamine-afgifte uitlokken. Invasie van de caudale vena cava: hoog bloedingsrisico.

Fase 3: postoperatieve intensive care

Plotselinge hormoondaling → mogelijke hypotensie.

  • Vasopressin ter ondersteuning van de bloeddruk indien nodig

Prognose

Voorzichtig; hangt af van metastasen en lokale invasie. Ongeveer 50% van de tumoren wordt als maligne beschouwd; sommige oncologische bronnen raden aan alle als maligne te behandelen.

Na geslaagde chirurgie en overleven van de kritieke fase kunnen overleving en kwaliteit van leven uitstekend zijn.

Preventie

Geen specifieke preventie. Vroege detectie via seniorcontroles: bloeddruk, bloedonderzoek, proteïnurie, screening-echo.

  • Collaps, flauwvallen of extreme zwakte
  • Plotselinge blindheid, wijde pupillen of oogbloeding
  • Onverklaarde neusbloeding
  • Ernstig hijgen of ademnood
  • Extreme onrust of zeer snelle hartslag
