Wat is het?

Ook bekend als insufficiëntie of ziekte van de craniële kruisband (CCL): een van de meest voorkomende orthopedische aandoeningen. De CCL ligt in de knie en is analoog aan de menselijke voorste kruisband (ACL). Hij verbindt femur en tibia en voorkomt craniaal schuiven van de tibia, endorotatie en overstrekking.

Bij partiële of complete ruptuur verliest de knie stabiliteit; ontsteking en pijn volgen. Bij honden is het meestal degeneratief: de band verzwakt tot dagelijkse activiteit de scheur uitlokt. Bij katten minder vaak; trauma, obesitas of leeftijdsdegeneratie kunnen een rol spelen.

Oorzaken en risicofactoren

An alternative theory is that lymphoplasmacytic synovitis is a primary immune-mediated disorder that causes joint laxity and instability, eventually leading to rupture of the cranial cruciate ligament. Some investigators have estimated that perhaps as many as 10% to 25% of cruciate ruptures in dogs are caused by this immunologic disorder, but this is a controversial claim.

Signalen

  • Kreupelheid achterpoot (cardinaal): aanhoudend, intermitterend of met opgetrokken poot.
  • Knie-instabiliteit (cardinaal): het gewricht geeft mee.
  • Pijn (vaak): verzet bij manipulatie; “sit test”.
  • Gewrichtseffusie (vaak): gezwollen knie.
  • Mediale buttress (soms): fibreuze verdikking mediaal bij chronische gevallen.

Hoe dierenartsen diagnosticeren

  • Palpatie van de knie (effusie, pijn, verdikking).
  • Cranial drawer en tibiacompressietest (soms met lichte sedatie).
  • Röntgen: effusie, artrose, andere botziekten.
  • MRI, echo of synoviale analyse bij twijfel.
  • Artroscopie: gouden standaard voor band en menisci.

Diagnosis of cranial cruciate ligament insufficiency is based on historical and clinical evidence of rear limb lameness. Palpation of the stifle is the most useful tool to make a diagnosis of cranial cruciate ligament insufficiency.

Behandelopties

Chirurgische stabilisatie (eerste lijn)

Aanbevolen voor de meeste honden bij partiële of complete ruptuur.

  • TPLO: verandert de biomechanica zodat de CCL niet meer nodig is bij belasting.
  • Extracapsulaire hechting: vaak bij kleine honden en katten.
  • Meniscusbeoordeling: een gescheurde meniscus moet mee behandeld worden.

Medisch (tweede lijn)

Als chirurgie onmogelijk is of primaire immuungemedieerde synovitis wordt vermoed:

  • Glucocorticoïden en zo nodig Azathioprine.
  • Conservatief: strikte rust, NSAID's, gewrichtssupplementen en revalidatie; soms voldoende bij zeer kleine honden (<15 kg) en katten.

Because rupture of the cranial cruciate ligament results in progressive degenerative joint changes within the joint, most such injuries should be repaired. In the authors' experience, many large breed dogs present with partial cranial cruciate ligament tears. The authors currently recommend repair in these cases even though they may not be grossly unstable.

Prognose

Met tijdige chirurgie en goede nazorg meestal gunstig tot uitstekend; velen keren binnen 3 tot 4 maanden terug naar pijnvrije activiteit. Bij ernstige pre-existente artrose: voorzichtiger. 40% tot 60% scheurt later ook de andere knie, vaak binnen 1 tot 2 jaar.

Preventie

  • Ideaal lichaamsgewicht behouden.
  • Regelmatige matige dagelijkse beweging.
  • Verantwoord fokken met aandacht voor gewrichtsgezondheid.

Wanneer de dierenarts bellen

Bij aanhoudende kreupelheid, stijfheid bij opstaan of onwil om te rennen en springen.

Spoed: plotseling volledig niet belasten, hevige pijn bij aanraking of onvermogen op beide achterpoten te staan.

Voor specifieke rassen

Bij Rottweiler, Newfoundlander, Staffordshire Bull Terrier of Labrador Retriever extra waakzaam zijn. Groei volgen, slank houden en bij eerste stijfheid van de achterhand de dierenarts raadplegen.
