TL;DR. La tetralogía de Fallot es un defecto cardiaco congénito raro y complejo con cuatro anomalías estructurales que limitan el oxígeno corporal y provocan intolerancia al ejercicio, mucosas azuladas y debilidad.

Cuatro anomalías interconectadas restringen mucho el aporte de oxígeno.

Qué es

ToF se debe a una desviación anterior del septo conal e incluye: (1) estenosis pulmonar, (2) CIV grande, (3) hipertrofia derecha, (4) aorta cabalgada. La sangre desaturada shuntea de derecha a izquierda evitando los pulmones (shunt D-I) con hipoxemia crónica. La ICC es infrecuente porque el shunt descarga presión:

Causas y factores de riesgo

Congénito, no por el entorno. Keeshond: herencia poligénica; English Bulldog: predisposición. En gatos, esporádico.

Signos

Frecuentes: cianosis, intolerancia al ejercicio, disnea, síncope, debilidad de esfuerzo, retraso de crecimiento, hipoxemia arterial, eritrocitosis (PCV a menudo >65–70 %). Ocasionales: convulsiones, soplos de eyección/holosistólicos, thrill precordial, pulsos yugulares, muerte súbita.

Diagnóstico

Auscultación, cianosis. Eco (estándar de oro) + Doppler; estudio de contraste para shunt D-I:

Hemograma (eritrocitosis), gases arteriales, radiografías (subcirculación pulmonar), ECG, a veces catéter.

Tratamiento

Objetivos: síntomas, viscosidad sanguínea, oxigenación.

  • Eritrocitosis: hidroxicarbamida (hidroxiurea) a dosis baja; flebotomía de urgencia si signos neurológicos.
  • Betabloqueantes (propranolol/atenolol) frente a obstrucción dinámica («tet spells»).
  • Shunts paliativos (Blalock-Taussig modificado) aumentan el flujo pulmonar:

"Palliative surgical procedures can increase pulmonary blood flow by creating a left-to-right shunt. Anastomosis of a subclavian artery to the pulmonary artery..."

  • Reparación completa con CEC: raramente disponible.

Pronóstico

Reservado; estenosis leve o shunt paliativo exitoso puede permitir edad media. Muerte súbita e hipoxemia progresiva siguen siendo riesgos. Gato: datos limitados.

Prevención

Sin prevención de estilo de vida. No criar afectados, padres ni hermanos de camada, sobre todo keeshond y bulldog inglés.

Cuándo llamar al veterinario

Urgencia: síncope, convulsiones/debilidad grave, mucosas azul oscuro/grises, disnea extrema, letargo/anorexia súbitos.

Para razas concretas

Keeshond: rasgo poligénico; cardiología de cría con eco. Bulldog inglés: evaluar pronto fatiga rápida o cianosis leve.
