Disseminated intravascular coagulation (DIC) is a life-threatening, secondary clotting disorder in dogs and cats. Triggered by severe underlying diseases like sepsis or trauma, it causes widespread micro-clots and severe bleeding, requiring immediate emergency veterinary intervention.

TL;DR. DIC is een levensbedreigend stollingssyndroom bij ernstige ziekte: microtrombi in kleine vaten én oncontroleerbare bloedingen door verbruik van stollingsfactoren.

Patiënten met DIC hebben intensieve ziekenhuisopname en continue support nodig.

Wat is het?

DIC is geen primaire ziekte maar een secundair syndroom. Het activeert massaal de stolling in de microcirculatie (microtrombi), verbruikt trombocyten en anticoagulantia en leidt dan tot bloedingen en orgaanfalen. Fibrinolyse loopt tegelijk op.

Oorzaken en risicofactoren

Altijd secundair. Vaak sepsis en SIRS; ook trauma, pancreatitis, gevorderde tumoren (bijv. hemangiosarcoom), hitteberoerte, ernstige infecties, slangenbeet. Greyhounds kunnen atypische stollingsprofielen hebben.

Signalen

  • Anemie (vaak): bleke slijmvliezen, zwakte.
  • Petechiën, ecchymosen (soms)
  • Epistaxis, tandvleesbloeding, GI- of urinebloeding (soms)
  • Langdurig bloeden op punctieplaatsen (soms)
  • Intracavitaire bloeding, hematomen (soms)
  • Aritmieën, orgaantekenen (soms)

Diagnostiek

Kliniek plus lab: trombocyten en uitstrijk (schistocyten, trombocytopenie), PT/aPTT, FDP/D-dimeer, antitrombine, eventueel TEG; biochemie en urine voor trigger en organen.

"These include a regenerative hemolytic anemia (although occasionally, because the animal has a chronic disorder such as cancer, the anemia is non regenerative), hemoglobinemia (caused by intravascular hemolysis), RBC fragments or schistocytes, thrombocytopenia, neutrophilia with a left shift, and rarely neutropenia."

Behandeling

Directe intensive care en behandeling van de oorzaak.

  • Vloeistoffen en expanders: i.v. vloeistoffen; eventueel Dextran 70 voor microcirculatie.
  • Anticoagulatie: ongefractioneerd Heparin voorzichtig en gemonitord.
  • Antiaggregatie: laaggedoseerde Aspirin in geselecteerde gevallen.
  • Heparine-antidotum: Protamine Sulfate bij heparinebloeding.

"Once a diagnosis of DIC has been established, or even if the degree of suspicion is high that DIC is present, treatment should be instituted without delay. Unfortunately, no controlled clinical trials have been performed in veterinary medicine evaluating the effects of different treatments in dogs with DIC, so this discussion reflects my recommendations for the treatment of dogs with this condition."

Prognose

Ernstig. Mortaliteit bij honden ~54 %; bij katten zeer hoog. Overleving hangt af van controle van de trigger.

Preventie

Geen directe preventie. Sleutel: vroege agressieve behandeling van ziekten met hoog DIC-risico.

Wanneer de dierenarts bellen

Direct bij: spontane bloeding; petechiën/ecchymosen; collaps; bleke slijmvliezen; dyspnoe; bloed in urine, feces of braaksel.

Voor specifieke rassen

Greyhound: atypische stolling; bij ernstige ziekte of chirurgie proactieve stollingspanelen overwegen.
