Qu'est-ce que c'est ?

Tumeur maligne des kératinocytes de la peau et des muqueuses. Formes cutanée (ulcères non cicatrisants) et orale (très invasive, souvent ostéolytique). Chiens et chats surtout; oiseaux et tortues possibles.

Causes et facteurs de risque

UV chroniques: principal facteur cutané. Risque: pelage blanc, peau claire, poils clairsemés, altitude, fort ensoleillement. Rôle possible d'un papillomavirus chez le chat.

Older, white cats or those with lightly pigmented noses and that live in sunny climates are at risk. Lesions progress slowly through early solar damage with crusting and erythema to carcinoma in situ to invasive squamous cell carcinoma."

"Sunlightinduced tumors on the flank and ventrum occur most commonly in lightly pigmented dog breeds such as Dalmatians, Beagles, Whippets, and White English Bull terriers. The condition is common in cats, with highest incidence reported in older white cats."

Signes à surveiller

Peau: croûtes oreilles/nez/paupières/ventre; érythème; ulcères non cicatrisants; masses verruqueuses.

Bouche: dysphagie, anorexie, haleine fétide, saignement.

Doigts: orteil gonflé douloureux; ongle déformé ou manquant sans traumatisme.

Diagnostic

Examen cutané, buccal et ganglionnaire. Cytologie souvent non concluante. Biopsie histopathologique: gold standard. Stadification: ponction ganglionnaire, radio thorax 3 vues, TDM orale.

Traitement

Chirurgie: large excision (pinnectomie, amputation digitale; orale: mandibulectomie/maxillectomie partielle).

Radiothérapie ± hyperthermie si résection incomplète.

Médical: Carboplatin, Cisplatin; Piroxicam palliatif.

Pronostic

SCC cutané précoce opéré: bon à excellent. SCC oral, surtout chat: réservé à sombre. Exotiques: données limitées.

Prévention

  • Limiter soleil 10:00-16:00
  • Crème solaire vétérinaire uniquement
  • Films anti-UV
  • Contrôles réguliers peau et bouche

Quand appeler le vétérinaire

Urgence: saignement buccal sévère, dysphagie brutale, amaigrissement rapide, douleur extrême.
