Was ist das?

Maligner Tumor der Plattenepithelzellen (Keratinozyten) von Haut und Schleimhaut. Zwei Formen: kutanes SCC (destruktive, nicht heilende Läsionen) und orales SCC (aggressiv, oft kieferzerstörend). Kutan meist langsamer; oral lokal hoch invasiv. Hunde und Katzen am häufigsten; auch Vögel und Schildkröten möglich.

Ursachen und Risikofaktoren

Chronische UV-Exposition ist der wichtigste Umweltfaktor für kutanes SCC. Risiko: weißes Fell, helle Haut, spärliche Behaarung, Hochlage, intensives Sonnenlicht. Bei Katzen wird Papillomavirus diskutiert.

Lesions progress slowly through early solar damage with crusting and erythema to carcinoma in situ to invasive squamous cell carcinoma."

"Sunlightinduced tumors on the flank and ventrum occur most commonly in lightly pigmented dog breeds such as Dalmatians, Beagles, Whippets, and White English Bull terriers. The condition is common in cats, with highest incidence reported in older white cats."

Anzeichen

Haut: Krusten an Ohrspitzen, Nase, Lidern oder Bauch; Rötung; nicht heilende Ulzera; warzenartige Wucherungen.

Mund: Dysphagie, Anorexie, Foetor, Maulblutung.

Zehen: geschwollene schmerzhafte Zehen; verformte oder fehlende Kralle ohne Trauma.

Diagnose

Untersuchung von Haut, Maul und Lymphknoten. Zytologie oft nicht beweisend. Biopsie mit Histopathologie ist Goldstandard. Staging: Lymphknoten-Aspiration, Thorax-Röntgen in 3 Ebenen, CT bei oralen Tumoren.

Behandlung

Chirurgie: Wahltherapie mit weiten Rändern (Pinnektomie, Zehenamputation; oral partielle Mandibulektomie/Maxillektomie).

Bestrahlung ± Hyperthermie bei unvollständiger Resektion.

Medikamentös: Carboplatin, Cisplatin; Piroxicam palliativ.

Prognose

Kutanes SCC früh und radikal entfernt: gut bis exzellent. Orales SCC, v. a. Katze: zurückhaltend bis schlecht. Exoten: begrenzte Daten.

Vorbeugung

  • Sonne 10:00-16:00 meiden bei hellen/spärlich behaarten Tieren
  • Nur petsichere Sonnencreme (kein Human-Zinkoxid/Salicylate)
  • UV-Schutzfolien an Fenstern
  • Regelmäßige Haut- und Maulkontrollen

Wann den Tierarzt rufen

Notfall: starke Maulblutung, plötzliche Schluckstörung, rascher Gewichtsverlust, extreme Schmerzen.
