¿Qué es?
También llamada enfermedad por almacenamiento de cobre o hepatitis crónica asociada al cobre: acumulación excesiva de cobre en los hepatocitos. El cobre es un mineral traza esencial de la dieta; el hígado lo procesa y elimina el exceso por la bilis. Si esa vía falla (defecto genético u otra enfermedad hepática), los niveles se vuelven tóxicos.
El cobre en exceso causa daño oxidativo, hepatitis crónica, fibrosis y, a veces, cirrosis irreversible. En raras ocasiones, necrosis hepática fulminante aguda. Se describe sobre todo en perros; es poco frecuente en gatos. La intervención temprana es clave.

El hígado procesa el cobre y excreta el exceso a través de la bilis.
Causas y factores de riesgo
Primaria: trastorno genético. En el Bedlington Terrier, una mutación del gen COMMD1 impide la excreción de cobre.
Secundaria: colestasis por otra enfermedad hepática que bloquea el flujo biliar.
Razas predispuestas:
- Bedlington Terrier: alta predisposición; autosómica recesiva.
- Labrador Retriever: predisposición genética sospechada.
- Dóberman: a menudo fase subclínica larga.
- Dálmata, West Highland White Terrier, Skye Terrier: predisposición sospechada.
"True copper storage disease likely represents a genetic defect in copper transport and/or storage, but the only breed in which this has been defined is the Bedlington Terrier. In this breed it is inherited as an autosomal recessive trait, and up to 60% of Bedlington Terriers in some countries have been affected in the past, although the prevalence is now decreasing as a result of selective breeding."

Opciones de tratamiento
Objetivos: eliminar cobre, evitar nueva acumulación y proteger el hígado.
- Penicilamina: quelante de primera línea; actúa en semanas o meses, no en crisis agudas.
- Trientina: alternativa si no se tolera la penicilamina.
- Ursodiol: ayuda al flujo biliar.
- Vitamina A: solo bajo supervisión estricta.
- Dieta baja en cobre y rica en zinc.
"Penicillamine is not helpful in an acute crisis because chelation takes weeks to months. However, it should be noted that there is much less information available about the pharmacokinetics, drug interactions, and toxicity of trientine in dogs than there is for d-penicillamine. Reported adverse effects include nausea, gastritis, abdominal pain, melena, and weakness."
Pronóstico
En fase asintomática o subclínica, con manejo de por vida: bueno a lentamente progresivo. Con falla hepática, cirrosis avanzada o necrosis fulminante: malo.
"The prognosis is poor, and most animals die within a few days [in acute crises]. Fortunately, this is uncommon; most dogs have a more chronic, protracted course, with several years of copper buildup and persistently high ALT activity, culminating in the development of chronic hepatitis with piecemeal necrosis, inflammation, and bridging fibrosis."
Prevención
- Cribado genético en Bedlington antes de criar.
- Controles anuales de ALT en razas predispuestas.
- Evitar alimentos ricos en cobre (vísceras, mariscos, legumbres); analizar el agua del grifo.
Cuándo llamar al veterinario
Ante vómito ocasional, menor apetito o letargo sin causa clara.
Urgencia: vómito intenso súbito, ictericia, debilidad extrema, heces alquitranadas (melena) o cambios neurológicos (desorientación, presión de la cabeza).
Para razas concretas
- Bedlington Terrier: la prueba genética es la herramienta clave.
