ما هو؟
عيب إنزيمي وراثي متنحي جسدي: لا تنتج الكريات الحمراء ما يكفي من ATP عبر التحلل السكري فتتحلل مبكراً. تجديد تعويضي ثم تليف/تصلب نخاع العظم وأذية كبدية من الحديد.

الأسباب وعوامل الخطر
وراثي فقط. الحاملون أصحاء. المصابون لديهم نسختان من الطفرة. تظهر العلامات غالباً في البلوغ المبكر. مرتبط بسلالات عديدة من الكلاب والقطط.
العلامات
- فقر دم (أساسي)
- خمول، شحوب، ضعف تحمل الجهد، نشاط أقل (شائع)
- تليف/تصلب النخاع (شائع ومتدرج)
- يرقان؛ فشل كبد من داء ترسب الحديد (أحياناً)
اللثة الشاحبة علامة مفتاحية لفقر الدم.
التشخيص
تعداد دم كامل (تجديد ثم غير تجديد)؛ اختبارات إنزيم في مختبرات متخصصة؛ الحمض النووي المعيار الذهبي للمصابين والحاملين.
الاختبار الجيني هو المعيار الذهبي.
العلاج
لا شفاء. دعم: نقل دم في الأزمة؛ إدارة الحديد بحذر؛ استئصال الطحال حسب الحالة؛ زراعة نخاع نادرة التطبيق.
التوقعات
الكلاب: حذرة إلى سيئة؛ غالباً تتدهور بين 3–5 سنوات. القطط: أكثر تبايناً؛ بعضها يعيش سنوات مع فقر دم مزمن تحت المتابعة.
الوقاية
فحص جيني قبل التربية؛ لا تزاوج بين حاملين.
متى تتصل بالطبيب
طوارئ عند الانهيار، صعوبة التنفس، اليرقان، بول داكن أو رفض الأكل/الشرب.
