Was ist das?

Maligne Lymphozytenerkrankung aus soliden Organen (Lymphknoten, Leber, Milz, Auge), im Gegensatz zur lymphatischen Leukämie aus dem Knochenmark. Formen: multizentrisch (Hund häufig), alimentär (Katze häufig), mediastinal, extranodal (Haut, Auge, Niere, ZNS).

Ursachen und Risikofaktoren

Multifaktoriell. Katze: FeLV/FIV-Assoziation. Umwelt (Passivrauch, Pestizide) verdächtigt. Mittelalter bis älter.

Anzeichen

  • Lymphadenopathie (kardinal, multizentrisch)
  • Lethargie, Dyspnoe/Husten, Gewichtsverlust, Anorexie, Erbrechen/Durchfall (häufig)
  • PU/PD (Hyperkalzämie), Augen- und Hautveränderungen, Niesen, Neurozeichen (gelegentlich)

Diagnose

FNA-Zytologie, Biopsie (Goldstandard), Ultraschall (Darmwand), Staging (CBC, Chemie, FeLV/FIV, Röntgen/US), Immunphänotyp (Flow/PARR).

Behandlung

Systemisch: Multiagenten-Chemo CHOP/CEOP (Goldstandard); COP Protocol als Alternative; Rescue z. B. Lomustine, Cytarabine (ZNS). Ziel: Lebensqualität und Remission.

Prognose

Unbehandelt: ca. 4–8 Wochen. Hund CHOP: ~80–90% Remission, ~12–16 Monate. Katze CHOP: ~65–75%, ~6–9 Monate. Small-cell intestinal (Katze): oft >2 Jahre.

Vorbeugung

Katze: Indoor, FeLV-Schutz. Weniger Rauch/Pestizide. Regelmäßige Untersuchung alter Tiere.

Dringend: Atemnot, plötzliche Blindheit, Kollaps, anhaltendes Erbrechen/Durchfall, Gesichtsödem, Fieber/Hypothermie.
