غالبا تورم غير مؤلم للعقد اللمفاوية المحيطية.
ما هو؟
خباثة لمفاوية من أعضاء صلبة (عقد، كبد، طحال، عين)، تختلف عن ابيضاض الدم اللمفاوي من النخاع. الأشكال: متعدد المراكز (شائع في الكلب)، هضمي (شائع في القط)، منصفي، خارج عقدي (جلد، عين، كلية، جهاز عصبي مركزي).
الأسباب وعوامل الخطر
متعددة العوامل. القط: ارتباط FeLV/FIV. البيئة (دخان، مبيدات) مشتبهة. متوسط العمر إلى كبار السن.
العلامات
- تضخم عقد (رئيسي، متعدد المراكز)
- خمول وضيق تنفس/سعال وفقدان وزن وفقد شهية وقيء/إسهال (شائعة)
- عطش/تبول زائد (فرط كالسيوم)، تغيرات عينية/جلدية، عطاس، علامات عصبية (أحيانا)
تغيرات عينية مثل التهاب العنبية قد ترافق المرض.
التشخيص
فحص خلوي بالشفط، خزعة (المعيار الذهبي)، موجات فوق صوتية (جدار الأمعاء)، تقييم المرحلة (تعداد دم، كيمياء، FeLV/FIV، أشعة/موجات)، النمط المناعي (flow/PARR).
الموجات تظهر تثخن جدار الأمعاء المميز لللمفوما الهضمية.
العلاج
جهازي: متعدد الأدوية CHOP/CEOP (مرجع)؛ COP Protocol بديل؛ إنقاذ Lomustine، Cytarabine (الجهاز العصبي). الهدف: جودة الحياة والهدأة.
التوقعات
بدون علاج: ~4–8 أسابيع. كلب CHOP: ~80–90% هدأة، ~12–16 شهرا. قط CHOP: ~65–75%، ~6–9 أشهر. أمعاء خلايا صغيرة (قط): غالبا >سنتين.
الوقاية
القط: داخل المنزل وحماية FeLV. أقل دخانا/مبيدات. فحوصات كبار السن.
متى تتصل بالطبيب البيطري
طارئ: ضيق تنفس، عمى مفاجئ، انهيار، قيء/إسهال مستمر، وذمة وجه، حمى/انخفاض حرارة.
