A portosystemic shunt (PSS) is an abnormal blood vessel that bypasses the liver, allowing toxins to build up in the bloodstream and causing neurological, digestive, and urinary issues in dogs and cats.

Wat is het?
Portaal bloed bereikt de systemische circulatie zonder leverpassering (aangeboren of verworven). Hepatische encefalopathie, groeiachterstand, uraatkristallen.
Oorzaken en risicofactoren
Aangeboren (kleine rassen vaak extrahepatisch; grote intrahepatisch). Verworven bij portale hypertensie.
Signalen
- Neurologische tekenen (cardinaal) na maaltijden
- Groeiachterstand, braken, diarree
- Polyurie/polydipsie, uraatkristallen
- Hypersalivatie (katten)
Kopdrukken is een veelvoorkomend neurologisch teken.
Diagnostiek
Galzuren, ammoniak, Doppler-echo, CT/angio; lab en urine.

Behandeling

Preventie
Aangedane dieren niet fokken; vroege diagnose verbetert resultaat.
Wanneer de dierenarts bellen
Pup met achterstand of neurotekenen postprandiaal. Spoed bij insulten of stupor.

Yorkie, maltezer, shih tzu en toys; grote rassen; ook katten.
