A portosystemic shunt (PSS) is an abnormal blood vessel that bypasses the liver, allowing toxins to build up in the bloodstream and causing neurological, digestive, and urinary issues in dogs and cats.

¿Qué es?

La sangre portal llega a la circulación sistémica sin pasar por el hígado (congénito extra/intrahepático o adquirido). Resulta en encefalopatía hepática, retraso del crecimiento y cristales de urato.

Causas y factores de riesgo

Congénito (razas pequeñas a menudo extrahepático; grandes intrahepático). Adquirido por hipertensión portal en hepatopatías crónicas.

Signos a vigilar

  • Signos neurológicos (cardinal): desorientación, presión de cabeza, convulsiones tras comidas
  • Retraso del crecimiento, vómitos, diarrea
  • Poliuria/polidipsia, orina con cristales de urato
  • Hipersalivación (gatos)

Cómo lo diagnostican los veterinarios

Bilisácidos pre/postprandiales, amonio, ecografía Doppler, TC/angiografía; a veces gammagrafía. Analítica y orina.

Opciones de tratamiento

Médico: dieta baja en proteínas de alta calidad, lactulosa, antibióticos intestinales. Quirúrgico: atenuación del shunt (mejor opción cuando es factible).

Pronóstico

Bueno a excelente tras cirugía exitosa en congénitos seleccionados. Reservado si múltiples shunts adquiridos o daño hepático grave.

Prevención

No hay prevención del congénito. Evitar cría de afectados. Diagnóstico precoz mejora resultados.

Cuándo llamar al veterinario

Cachorro con retraso, signos neurológicos postprandiales o cristales de urato. Urgencia si convulsiones, estupor o colapso.

Para razas específicas

Yorkie, maltes, shih tzu y otras toy (extrahepático); razas grandes (intrahepático); también gatos.
