TL;DR. Aggressiver Blutgefäßkrebs am Herzen: plötzliche Blutung, Herztamponade, Notfall. Prognose vorsichtig.

Meist rechtes Atrium; Blut sammelt sich im Perikard.

Was ist das?

Hochmaligner Endothelzell-Tumor. Beim Hund häufig am rechten Atrium/Aurikel. Ruptur füllt den Perikardsack (hämorrhagischer Erguss) und kann zur Tamponade führen: das Herz kann sich nicht füllen.

Ursachen und Risikofaktoren

Ursache unklar; genetische Disposition. Mittelalte bis ältere Hunde. Bei Herzbefund immer auch Milz und Leber prüfen.

"Approximately 50% of the tumors originate in the spleen, 25% in the right atrium, 13% in subcutaneous tissue, 5% in the liver, 5% in the liver-spleen-right atrium, and 1% to 2% simultaneously in other organs (i. e. , kidney, urinary bladder, bone, tongue, prostate)."

Behandlung

Notfallstabilisierung

Perikardiozentese entlastet die Tamponade.

Chirurgie

Aurikelresektion und/oder partielle Perikardektomie (palliativ).

Chemotherapie (VAC)

Vincristine, Doxorubicin, Cyclophosphamide

"Sixty-seven dogs with HSA in different anatomic locations were evaluated retrospectively. All dogs received the VAC protocol, as adjuvant to surgery ( n = 50), neoadjuvant ( n = 3), or as the sole treatment modality ( n = 14). Supportiv

glucocorticoids palliativ möglich.

Prognose

Ohne Therapie oft nur Tage bis Wochen. Chirurgie + VAC: median ca. 189-195 Tage. Katzen: Daten begrenzt. Fokus Lebensqualität.

Vorbeugung

Keine bekannte Prävention. Bei Risikorassen ggf. Screening-Echo ab mittlerem Alter; unauffälliger Befund garantiert nichts.
