¿Qué es?

Síndrome clínico: el corazón no aporta el volumen de sangre necesario. Sube la presión venosa y se filtra líquido (congestión). Falla izquierda: edema pulmonar. Falla derecha: ascitis o derrame pleural. La retención de sodio y agua y la taquicardia compensan al inicio, pero sobrecargan más el corazón.

Causas y factores de riesgo

Perros: enfermedad valvular degenerativa y miocardiopatía dilatada (MCD). Gatos: miocardiopatía hipertrófica (MCH).

Signos a vigilar

  • Cardinal: edema pulmonar
  • Frecuentes: tos (perros), disnea, taquipnea (reposo >30-40/min es alarma), derrame pleural, ascitis, intolerancia al ejercicio, debilidad
  • Ocasionales: distensión yugular, hepatomegalia, síncope, caquexia cardíaca
  • Raros: shock cardiogénico

Diagnóstico

Auscultación (soplo, galope, crepitantes), mucosas, pulso.

  • Radiografía de tórax: cardiomegalia, edema o derrame
  • Ecocardiografía: causa estructural
  • ECG / Holter: arritmias

Tratamiento

Pronóstico

Depende de la causa, el estadio y la respuesta. Antes de la ICC se puede controlar mucho tiempo; con ICC activa el pronóstico es más reservado. Muchos disfrutan meses o años con buena calidad de vida.

Prevención

Controles periódicos, screening en razas de riesgo (eco, Holter, genética), peso saludable y nutrición adecuada (p. ej. taurina).

Cuándo llamar al veterinario

Urgencia: dificultad respiratoria grave, taquipnea en reposo, respiración bucal (gatos), síncope, mucosas pálidas/azuladas, parálisis posterior súbita (gatos: trombo).
Urgente (24 h): tos nueva o peor, anorexia, abdomen hinchado, menos energía.

Razas específicas

Dóberman: ECG y Holter anuales antes de signos. Irish Wolfhound: vigilar fibrilación auricular. Maine Coon: test genético de MCH y eco basal.
