Wat is het?
Degeneratieve lumbosacrale stenose vernauwt het kanaal bij L7–sacrum, waar de cauda-equina-wortels lopen. Compressie geeft pijn, ontsteking en neurologische uitval.
De cauda equina is een bundel zenuwwortels in het lage kanaal.
Oorzaken en risicofactoren
Vooral degeneratief: discus L7–S + instabiliteit + verdikking van facetten, kapsels en ligamentum flavum. Vaak bij grote actieve honden; predispositie bij German Shepherd Dogs, Belgian Malinois en Labrador Retrievers.
Signalen
Cardinal
- Pijn bij diepe palpatie van het sacrum; pijn bij dorsaalflexie van de staart of hyperextensie lendenen
Vaak
- Weigering te rennen, springen, trappen; achterhandkreupelheid; traag opstaan
Soms / rood
- Achterhandzwakte, lage staart, atrofie, afwijkende reflexen; verlaagde anale tonus, incontinentie, zelftrauma perineum

Diagnose
Uitsluiten van discospondylitis, prostaatziekte en artrose. Klinische tests (staart, lordose). Röntgen. MRI in extensie gouden standaard.
MRI in extensie: gouden standaard bij compressie.
Moeite met opstaan na rust: vaak een vroeg teken.
Behandeling
Medisch (eerstelijn bij mild)
- Strikte rust; Gabapentin; Tramadol; NSAID mogelijk
Chirurgisch
- Decompressieve laminectomie bij falen van medicatie of progressieve deficitten
Prognose
Alleen kreupelheid en milde deficitten: vaak uitstekend. Incontinentie of zware deficitten: gereserveerd tot slecht. Chirurgisch succes op lange termijn ~55 %.
Preventie
Ideaal gewicht, low-impact beweging, vroege check bij risicorassen.
Wanneer de dierenarts bellen
Afspraak bij traag opstaan, traponwil of lage staart. Spoed bij plotselinge achterhandparese, incontinentie, extreme pijn of agressief likken van staartbasis/lies.
Voor specifieke rassen
Extra alert bij German Shepherd Dogs, Belgian Malinois en Labrador Retrievers.
