باختصار

Pleural effusion is a life-threatening accumulation of fluid around the lungs in dogs and cats. Recognizing signs like rapid breathing, increased effort, or an abnormal "paradoxical" breathing pattern is critical for seeking immediate veterinary emergency care.

ما هو؟

سائل غير طبيعي في الحيز الجنبي. يُصنَّف إلى رشاحة نقية/معدلة وإفراز حسب البروتين والخلايا. علامة لمرض كامن وليست مرضاً قائماً بذاته.

الأسباب وعوامل الخطر

فشل قلبي، تقيح صدر، أورام، كيلوتوراكس، دم في الصدر (رضح/سموم قوارض)، التهاب الصفاق المعدي السنوري (FIP) في القطط.

العلامات

  • تسرع تنفس، جهد تنفسي، ضيق تنفس
  • أصوات قلب ورئة مكتومة
  • تنفس متناقض، عدم تحمل الجهد، خمول
  • أغشية شاحبة أو مزرقة في الحالات الشديدة

التشخيص

فحص سريري، بزل صدري، تحليل السائل (المعيار الذهبي)، أشعة/موجات بعد التثبيت؛ CT أو جراحة عند الحاجة.

البزل يخفف التنفس ويوفر عينة تشخيصية.

العلاج

تصريف طارئ + أكسجين. حسب السبب: أنابيب/مضادات (تقيح صدر)، أدوية قلب/مدرات، حمية قليلة الدهن جداً/جراحة (كيلوتوراكس)، تلطيف (أورام)، جراحة (رضح).

المآل

يعتمد على السبب: تقيح صدر جيد إلى ممتاز؛ أورام غالباً سيئ؛ قلب/كيلوتوراكس حذر إلى متوسط.

الوقاية

فحوصات دورية، قطط داخل المنزل، تجنب الأعشاب الشائكة، لا تتجاهل تغيرات التنفس.

متى تتصل بالطبيب البيطري

طارئ عند التنفس بفم مفتوح، >40 نفساً/دقيقة أثناء الراحة، تنفس بطني، أغشية غير طبيعية أو انهيار.
