Feline hepatic lipidosis, commonly known as fatty liver disease, is a serious, life-threatening metabolic condition where fat builds up rapidly in a cat's liver cells. Often triggered by a period of decreased appetite, this condition requires prompt veterinary intervention. Fortunately, with early diagnosis and aggressive, long-term nutritional support, the disease is highly reversible, and most cats can make a full recovery.

¿Qué es?

Hepatopatía frecuente y grave: acumulación rápida de triglicéridos en hepatocitos con riesgo de fallo agudo. El gato moviliza grasa al hígado si deja de comer; sin proteínas no la procesa (colestasis).

Causas y factores de riesgo

Primaria: anorexia sin causa clara (estrés).
Secundaria: pancreatitis, diabetes, otras hepatopatías, EII, neoplasia.
Riesgo: obesidad; 2–7 días de anorexia pueden bastar.

Cardinales

  • Anorexia
  • Icterus

Frecuentes

  • Pérdida de peso y músculo
  • Hepatomegalia, vómitos, deshidratación

Ocasionales

  • Ptialismo, letargia
  • Encefalopatía hepática
  • Estreñimiento o diarrea

Diagnóstico

Historia y exploración. Bioquímica: hiperbilirrubinemia (>95%), ALT/AST/ALP altas; GGT normal o poco elevada. Eco hiperecogénica; PAAF; histopatología (biopsia en cuña) de referencia.

Tratamiento

Nutrición central. Evitar jeringa forzada. Preferible sonda de esofagostomía; introducción lenta; 4–8 semanas. Fluidos IV y electrolitos; antieméticos; analgesia; lactulosa si encefalopatía.

Pronóstico

Con sonda precoz, recuperación a menudo del 80–90%. Cirrosis crónica rara.

Prevención

Sin dietas drásticas; vigilar ingesta; reducir estrés; si no come >24–48 h, veterinario.

Cuándo llamar al veterinario

Urgencia: ictericia; letargia extrema; vómitos persistentes; babeo; desorientación, head pressing o convulsiones.
