Hva er det?
Septisk eksudat i pleurahulen. Volumet komprimerer lungen og gir åndenød. Systemisk betennelse og sepsis mulig. Høyeste hast.
Årsaker og risikofaktorer
Katt ofte idiopatisk (orofaryngeale bakterier mistenkes). Hund: traume, plantefremmedlegemer, bitt, pneumoni med gjennombrudd. Ingen dokumentert rasepredisposisjon.
Tegn
- Takypné
- Markert bukpusting
- Dempte lungelyder
- Feber, letargi, anoreksi
- Vekttap, dehydrering; mulig SIRS/septisk sjokk
Anstrengt pust med strukket hals og åpen munn er akuttegn.
Diagnostikk
Thoraxrøntgen, cytologi av væske (gullstandard), Gram, aerob/anaerob dyrkning, ultralyd, CT ved mistanke om fremmedlegeme.
Ultralyd lokaliserer væskelommer og styrer drenasje.
Behandling
Thoraxdren, skylling, i.v. bredspektrede antibiotika, væske. Oralt 4–6 uker. Kirurgi ved terapisvikt eller fremmedlegeme/abscess.
Prognose
Moderat til god ved tidlig aggressiv terapi; katter med dren ofte god overlevelse. Første dager kritiske.
Forebygging
Unngå høyt gress/agn; slåssing; behandle sår og hoste tidlig; tannhelse (katt).
Når ringe veterinæren
Umiddelbart ved åndenød, cyanose, kollaps eller høy feber med anoreksi.
