Qu'est-ce que c'est ?
Exsudat septique pleural. Le volume comprime le poumon et cause une détresse respiratoire. Inflammation systémique et sepsis possibles. Urgence maximale.
Causes et facteurs de risque
Chat souvent idiopathique (bactéries oropharyngées suspectées). Chien: traumatisme, corps étrangers végétaux, morsures, pneumonie perforante. Pas de prédisposition raciale documentée.
Signes à surveiller
- Tachypnée
- Respiration abdominale marquée
- Bruits pulmonaires diminués
- Fièvre, léthargie, anorexie
- Amaigrissement, déshydratation; SIRS/choc septique possible
Respiration laborieuse, cou étendu et bouche ouverte: urgence.
Comment les vétérinaires diagnostiquent
Radiographies thoraciques, cytologie du liquide (référence), Gram, cultures aéro/anaérobies, échographie, TDM si corps étranger suspecté.
L'échographie localise les poches de liquide et guide les drains.
Options de traitement
Drains thoraciques, lavage, antibiotiques i.v. à large spectre, fluides. Orale 4–6 semaines. Chirurgie si échec médical ou corps étranger/abcès.
Pronostic
Moyen à bon si traitement précoce agressif; chats drainés souvent bon pronostic. Premiers jours critiques.
Prévention
Éviter herbes hautes/épillets; combats; traiter blessures et toux tôt; hygiène dentaire (chat).
Quand appeler le vétérinaire
Immédiatement si détresse respiratoire, cyanose, collapsus ou forte fièvre avec anorexie.
