Hvad er det?
Septisk eksudat i pleurahulen. Volumen komprimerer lungen og giver åndenød. Systemisk inflammation og sepsis mulige. Højeste hast.
Årsager og risikofaktorer
Kat ofte idiopatisk (orofaryngeale bakterier mistænkes). Hund: traume, plantefremmedlegemer, bid, pneumoni med gennembrud. Ingen dokumenteret raceprædisposition.
Tegn
- Takypnø
- Markant bugånding
- Dæmpede lungelyde
- Feber, letargi, anoreksi
- Vægttab, dehydrering; mulig SIRS/septisk shock
Anstrengt vejrtrækning med strakt hals og åben mund er akuttegn.
Diagnostik
Thoraxrøntgen, cytologi af væske (guldstandard), Gram, aerob/anaerob dyrkning, ultralyd, CT ved mistanke om fremmedlegeme.
Ultralyd lokaliserer væskelommer og styrer dræn.
Behandling
Thoraxdræn, skylning, i.v. bredspektrede antibiotika, væske. Oralt 4–6 uger. Kirurgi ved terapisvigt eller fremmedlegeme/absces.
Prognose
Moderat til god ved tidlig aggressiv terapi; katte med dræn ofte god overlevelse. Første dage kritiske.
Forebyggelse
Undgå højt græs/avner; slagsmål; behandle sår og hoste tidligt; tandsundhed (kat).
Hvornår ringe til dyrlægen
Straks ved åndenød, cyanose, kollaps eller høj feber med anoreksi.
