Apa itu?
Eksudat septik di rongga pleura. Volume menekan paru dan menyebabkan sesak. Radang sistemik dan sepsis mungkin. Urgensi tertinggi.
Penyebab dan faktor risiko
Kucing sering idiopatik (bakteri orofaringeal dicurigai). Anjing: trauma, benda asing tumbuhan, gigitan, pneumonia pecah. Tidak ada predisposisi ras yang didokumentasikan.
Tanda
- Takipnea
- Pernapasan perut menonjol
- Suara paru berkurang
- Demam, letargi, anoreksia
- Turun berat, dehidrasi; mungkin SIRS/syok septik
Napas dipaksakan dengan leher terjulur dan mulut terbuka adalah tanda darurat.
Diagnosis
Rontgen toraks, sitologi cairan (baku emas), Gram, kultur aerob/anaerob, ultrasonografi, CT jika curiga benda asing.
USG menemukan kantung cairan dan memandu drainase.
Pengobatan
Drain toraks, bilas, antibiotik i.v. spektrum luas, cairan. Oral 4–6 minggu. Bedah jika gagal atau ada benda asing/abses.
Prognosis
Sedang hingga baik dengan terapi agresif dini; kucing dengan drain sering bertahan baik. Hari pertama kritis.
Pencegahan
Hindari rumput tinggi/bulir; perkelahian; obati luka dan batuk lebih awal; kebersihan gigi (kucing).
Kapan menghubungi dokter hewan
Segera jika sesak napas, sianosis, kolaps, atau demam tinggi dengan anoreksia.
