Wat is het?
Septisch exsudaat in de pleuraholte. Volume comprimeert de long en veroorzaakt ademnood. Systemische ontsteking en sepsis mogelijk. Hoogste urgentie.
Oorzaken en risicofactoren
Kat vaak idiopathisch (orofaryngeale bacteriën vermoed). Hond: trauma, plantaardige vreemde lichamen, bijtwonden, pneumonie met doorbraak. Geen gedocumenteerde raspredispositie.
Signalen
- Tachypneu
- Uitgesproken buikademhaling
- Verminderde longgeluiden
- Koorts, lethargie, anorexie
- Gewichtsverlies, dehydratie; mogelijk SIRS/septische shock
Moeizame ademhaling met gestrekte nek en open bek is een noodsignaal.
Diagnose
Thoraxröntgen, cytologie van vocht (gouden standaard), Gram, aërobe/anaërobe kweek, echografie, CT bij verdenking vreemd lichaam.
Echo lokaliseert vochtzakken en stuurt drains.
Behandeling
Thoraxdrains, spoeling, i.v. breedspectrumantibiotica, vocht. Oraal 4–6 weken. Chirurgie bij falen of vreemd lichaam/abces.
Prognose
Matig tot goed bij vroege agressieve therapie; katten met drains vaak goede overleving. Eerste dagen kritiek.
Preventie
Hoog gras/aren vermijden; vechten; wonden en hoest vroeg behandelen; tandhygiëne (kat).
Wanneer de dierenarts bellen
Direct bij ademnood, cyanose, collaps of hoge koorts met anorexie.
