Addison's disease is an uncommon endocrine disorder where the adrenal glands fail to produce vital hormones. This leads to severe gastrointestinal issues, weakness, and life-threatening electrolyte imbalances. While an Addisonian crisis is a veterinary emergency, the long-term prognosis is excellent with lifelong daily or monthly hormone replacement therapy.

Hva er det?
For lite gluko- og mineralkortikoider. Hovedsakelig hunder; sjelden hos katt. Uten aldosteron: hyperkalemi, hyponatremi, hypovolemi, bradykardi, sjokk.
Årsaker og risikofaktorer
Ofte immunmediert barkdestruksjon. Også iatrogen etter brått steroidstopp eller overbehandling av Cushing. Rasepredisposisjon hos noen hunder.
Tegn å se etter
- Letargi, anoreksi, oppkast/diaré, svakhet (vanlige)
- Dehydrering; bradykardi (av og til)
- Kollaps / hypovolemisk sjokk (Addisonkrise)
Letargi og svakhet er vanlige tidlige tegn.
Diagnose
Na:K-forhold, hyperkalemi. ACTH-stimuleringstest (gullstandard). EKG ved K-arytmier.
EKG fanger farlige arytmier fra høyt kalium.
Behandling
Krise: aggressive i.v. væsker, Dexamethasone. Vedlikehold: DOCP eller Fludrocortisone pluss prednison; stressdoser.
Prognose
Utmerket ved stabil livslang terapi.
Forebygging
Aldri stopp steroider brått. Avlsscreening ved predisposisjon.
Når ringe veterinær
Akutt: kollaps, ekstrem svakhet, blodig diaré/oppkast, bleke slimhinner.
