Addison's disease is an uncommon endocrine disorder where the adrenal glands fail to produce vital hormones. This leads to severe gastrointestinal issues, weakness, and life-threatening electrolyte imbalances. While an Addisonian crisis is a veterinary emergency, the long-term prognosis is excellent with lifelong daily or monthly hormone replacement therapy.

Hvad er det?
For lidt gluko- og mineralkortikoider. Primært hunde; sjældent hos kat. Uden aldosteron: hyperkaliæmi, hyponatriæmi, hypovolæmi, bradykardi, shock.
Årsager og risikofaktorer
Ofte immunmedieret barkdestruktion. Også iatrogen efter abrupt steroidstop eller overbehandling af Cushing. Raceprædisposition hos nogle hunde.
Tegn at holde øje med
- Letargi, anoreksi, opkast/diarré, svaghed (hyppige)
- Dehydrering; bradykardi (lejlighedsvis)
- Kollaps / hypovolæmisk shock (Addison-krise)
Letargi og svaghed er hyppige tidlige tegn.
Diagnose
Na:K-ratio, hyperkaliæmi. ACTH-stimulationstest (guldstandard). EKG ved K-arytmier.
EKG fanger farlige arytmier fra højt kalium.
Behandling
Krise: aggressive i.v. væsker, Dexamethasone. Vedligehold: DOCP eller Fludrocortisone plus prednison; stressdoser.
Prognose
Fremragende ved stabil livslang terapi.
Forebyggelse
Stop aldrig steroider abrupt. Avlsscreening ved prædisposition.
Hvornår ringe til dyrlægen
Akut: kollaps, ekstrem svaghed, blodig diarré/opkast, blege slimhinder.
