Addison's disease is an uncommon endocrine disorder where the adrenal glands fail to produce vital hormones. This leads to severe gastrointestinal issues, weakness, and life-threatening electrolyte imbalances. While an Addisonian crisis is a veterinary emergency, the long-term prognosis is excellent with lifelong daily or monthly hormone replacement therapy.

Thượng thận nằm trước thận, tạo cortisol và aldosterone.
Là gì?
Thiếu gluco- và mineralocorticoid. Chủ yếu ở chó; mèo hiếm. Không có aldosterone: tăng kali, hạ natri, giảm thể tích, nhịp chậm, sốc.
Nguyên nhân và yếu tố nguy cơ
Thường phá hủy vỏ do miễn dịch. Cũng do iatrogen (ngừng steroid đột ngột hoặc điều trị Cushing quá mức). Một số giống có khuynh hướng.
Dấu hiệu cần theo dõi
- Uể oải, biếng ăn, nôn/tiêu chảy, yếu (thường gặp)
- Mất nước; nhịp chậm (đôi khi)
- Sụp / sốc giảm thể tích (cơn Addison)
Uể oải và yếu là dấu hiệu sớm thường gặp.
Chẩn đoán
Tỷ lệ Na:K, tăng kali. Test kích thích ACTH (tiêu chuẩn vàng). Điện tim nếu loạn nhịp do K.
Điện tim phát hiện loạn nhịp nguy hiểm do kali cao.
Điều trị
Cơn: dịch i.v. mạnh, Dexamethasone. Duy trì: DOCP hoặc Fludrocortisone kèm prednisone; liều stress.
Tiên lượng
Rất tốt với liệu pháp ổn định suốt đời.
Phòng ngừa
Không ngừng steroid đột ngột. Sàng lọc sinh sản nếu có khuynh hướng.
Khi nào gọi thú y
Cấp cứu: sụp, yếu cực, tiêu chảy/nôn ra máu, nướu nhợt.
