Qu'est-ce que c'est ?

La stomatite est une inflammation sévère de la cavité buccale. Contrairement à la gingivite (gencives seules), elle concerne lèvres, joues, langue, palais et gorge. Réponse immunitaire excessive au plaque ou à des facteurs systémiques : gonflement, ulcères, saignement, douleur extrême pour manger et se toiletter.

Termes : stomatite féline, lymphoplasmocytaire, ulcéroproliférative chronique.

Causes et facteurs de risque

  • Immunosuppression : FeLV, FIV, diabète, hyperadrénocorticisme.
  • Virus félins : calicivirus, herpèsvirus-1.

  • Réaction au plaque ; urémie ; traumatisme/caustiques.
  • Race : Bichon maltais prédisposé au CUPS.

Fréquents

  • Douleur buccale intense (cri, patte à la bouche, lâcher de croquettes)
  • Salive épaisse, filante, parfois sanglante
  • Haleine très forte
  • Anorexie malgré l'appétit

Occasionnels

  • Perte de poids, dysphagie, fièvre

  • Examen buccal sous anesthésie
  • Radiographies dentaires
  • Biopsie (exclure carcinome épidermoïde)
  • Bilan sanguin ; chez le chat tests FeLV/FIV

Options de traitement

Chirurgie

Extractions des prémolaires/molaires ou bouche complète pour supprimer le plaque et le stimulus immunitaire. Adaptation alimentaire généralement bonne.

Médical

Analgésie, anti-inflammatoires/immunosuppresseurs si infection exclue, détartrages professionnels si cas légers.

Pronostic

Variable. Formes métaboliques ou transitoires meilleures ; formes chroniques liées au FCV souvent réfractaires. Les extractions aident beaucoup d'animaux ; certains nécessitent un suivi à vie.

Prévention

Brossage quotidien, soins pro, vaccination FCV/FHV-1, traitement précoce de la gingivite.

Quand appeler le vétérinaire

Mauvaise haleine, salivation, changement d'appétit : consultation. Urgence si refus de boire/manger >24 h, douleur vocale au bol, saignement buccal abondant, fièvre avec prostration.

Races particulières

Maltais : CUPS avec lésions de contact sur les joues ; hygiène dentaire stricte.
