Pericardial effusion is a life-threatening veterinary emergency where fluid builds up in the sac surrounding the heart. This pressure prevents the heart from filling with blood, leading to weakness, breathing difficulties, and potential collapse. Væskeophobning i hjerteposen der komprimerer hjertet (tamponade) og sænker minutvolumen.
"Cardiac tamponade develops when pericardial fluid accumulation raises intrapericardial pressure to or above the normal diastolic pressure of the right side of the heart, preventing normal filling."
Årsager og risikofaktorer
Neoplasi (hæmangiosarkom, kemodektom) og idiopatisk hæmoragisk; store racer: golden, labrador, sankt bernhard, schæfer.
Tegn
- Dæmpede hjertelyde (kardinal)
- Dyspnø, anstrengelsesintolerance, ascites, blege slimhinder
- Kollaps, svag puls
Ascites afspejler højresidig stase.
Diagnostik
Ekokardiografi (guld); røntgen (globoitt hjerte); EKG (lav spænding, elektrisk alternans); væskeanalyse.
Ultralyd viser den komprimerende væske.
Behandling
Perikardiocentese akut. Ingen diuretika/vasodilatorer før drænage. Steroider ved idiopatisk; perikardektomi ved recidiv.
Prognose
Idiopatisk ofte god. Neoplastisk: forbeholden til dårlig.
Forebyggelse
Ingen specifik forebyggelse; kontroller hos risikoracer.
Hvornår ringe til dyrlæge
Dyspnø, kollaps, blege/blå slimhinder, spændt bug: akut niveau 5.
