TL;DR. Progressieve hartziekte: littekenweefsel verstijft de kamers, belemmert vulling en leidt vaak tot hartfalen of stolsels.

Fibrose verstijft de wanden; het linker atrium is vaak sterk vergroot.

Wat is het?

RCM is een zeldzame maar ernstige myocardziekte bij de kat. Anders dan HCM (dikke wand) is extreme stijfheid door fibrose het kernprobleem: diastolische dysfunctie, terugstroom naar het linker atrium, oedeem/pleuravocht en hoog trombo-embolierisico.

Based on this classification scheme, there are four basic types of cardiomyopathy: (1) dilated cardiomyopathy, (2) hypertrophic cardiomyopathy (HCM), (3) restrictive cardiomyopathy (RCM), and (4) arrhythmogenic right ventricular cardiomyopathy. All these forms are observed in the cat.

Oorzaken en risicofactoren

Idiopathisch, waarschijnlijk multifactorieel (endomyocarditis, eindstadium na HCM, infiltraten zoals lymfoom).

Restrictive cardiomyopathy (RCM) is associated with extensive endocardial, subendocardial, or myocardial fibrosis of unclear, but probably multifactorial, cause.

Middelbare tot oudere katten; geen gedocumenteerde raspredispositie.

Signalen

  • Dyspneu, tachypneu, open-mondademhaling (vaak)
  • Inactiviteit, anorexie, gewichtsverlies, braken (vaak)
  • Jugulaire stuwing (vaak)
  • Trombo-embolie / zadel-trombus (vaak en spoed)
  • Suizend geruis, galop, aritmie (soms)

Toegenomen ademarbeid is een waarschuwing voor congestief hartfalen.

Diagnose

Echocardiografie (gouden standaard): groot linker atrium, bijna normale wanddikte, stijve vulling, mogelijk trombus/smoke. Röntgen, ECG.

Behandeling

Geen genezing van fibrose. Diuretica (furosemide), ACE-remmers, antiplaatjes (clopidogrel), antiaritmica zo nodig. Rustademfrequentie thuis volgen.

Prognose

Voorzichtig tot slecht; progressief. Met vroege detectie maanden tot meer dan een jaar comfort mogelijk.

Preventie

Geen primaire preventie. Regelmatige controles; geruis/galop/aritmie uitzoeken.

Wanneer de dierenarts bellen

Direct bij rust-AF >30–40/min, open-mondademhaling, achterhandverlamming, koude poten, collaps of totale anorexie.
