Wat is het?
De patella glijdt in de trochlea van het femur. Luxatie mediaal (vaak) of lateraal. Graad I–IV. Mechanische kreupelheid en kraakbeenschade.
Oorzaken en risicofactoren
Meestal congenitaal (ondiepe groeve, malalignement); zelden traumatisch. Mediaal: kleine/toy rassen; lateraal: grote rassen, vaak met heupdysplasie. Kat minder vaak, meestal mediaal.
Signalen
- Huppelende gang
- Kreupelheid
- Varus (O-benen) bij mediaal
- Lage houding, tenen naar binnen
- Zelden: acuut niet belasten
O-benen zijn vaak bij mediale luxatie.
Diagnostiek
Palpatie en graad I–IV. Röntgen knie en heup: groeve, as, artrose, bijkomende aandoeningen.
Röntgen beoordeelt botas en heup.
Behandeling
Conservatief I / milde II: gewicht, low-impact beweging, gewrichtsondersteuning. Chirurgie symptomatische II–IV: trochleoplastiek, TTT, weke delen; osteotomieën bij IV.
Prognose
I–III na chirurgie vaak goed/uitstekend. IV gereserveerd tot slecht; artrose mogelijk.
Preventie
Niet fokken met aangedane dieren. Ideaal gewicht vanaf jong.
Wanneer de dierenarts bellen
Afspraak bij huppelen of O-benen. Spoed bij plots niet belasten, hevige pijn of niet opstaan.
