Canine distemper is a highly contagious, severe viral disease affecting dogs and ferrets. It attacks multiple body systems, including the respiratory, gastrointestinal, and nervous systems, often leading to irreversible neurological damage. TL;DR. Zeer besmettelijke, levensbedreigende virusinfectie bij hond en fret met schade aan luchtwegen, maag-darm en zenuwstelsel; vaak blijvende neurologische gevolgen.
Ernstige systemische ziekte met uitgesproken lethargie.
Wat is het?
Morbillivirus (CDV). Treft lymfoïd, nerveus en epitheelweefsel. Fasen: lymfopenie/koorts, dan luchtwegen/GI/huid/ogen, vaak CZS. Geen genezend antiviraal middel.
Oorzaken en risicofactoren
Aerosol en lichaamsvloeistoffen.
"CDV is shed in respiratory secretions for up to 90 days after infection. Initial replication of CDV is in lymphoid tissue, and viral destruction of lymphocytes results in lymphopenia and pyrexia."
Ongevaccineerde pups 6-12 weken hoogste risico; fretten bijna 100% fataal. Geen raspredispositie.
Signalen
- Myoclonus (cardinaal)
- Vaak: koorts, depressie, mucopurulente neusuitvloeiing, hoesten, braken/diarree, pustels, hyperkeratose (hard pad), insulten, ataxie
- Soms: uveïtis, optische neuritis, KCS, glazuurhypoplasie
Hyperkeratose van de voetzolen is klassiek.
Diagnose
CBC (lymfopenie), thoraxfoto, conjunctivale cytologie, RT-PCR (gouden standaard), IHC, eventueel liquor.
Thoraxfoto beoordeelt secundaire pneumonie.
Behandeling
Louter ondersteunend: IV-vloeistoffen, antibiotica tegen secundaire infecties, ademtherapie, anti-emetica, anticonvulsiva. Glucocorticoids selectief. Strenge isolatie.
Prognose
Vaak slecht, vooral bij CZS-tekenen.
"CNS disease is generally progressive and carries a poor prognosis; it can develop in some dogs that never had systemic signs of disease recognized."
Preventie
Core-vaccinatie pups + boosters; fret-specifiek vaccin; quarantaine; desinfectie.
Wanneer de dierenarts bellen
Spoed: insulten/myoclonus; ernstige ademnood; geelgroene uitvloeiing; collaps; ernstige GI-tekenen; plotselinge blindheid.
