Canine distemper is a highly contagious, severe viral disease affecting dogs and ferrets. It attacks multiple body systems, including the respiratory, gastrointestinal, and nervous systems, often leading to irreversible neurological damage. Maladie systémique grave et hautement contagieuse.
Qu'est-ce que c'est ?
Virus de la maladie de Carré (CDV): tissu lymphoïde → épithélium → souvent SNC. Lymphopénie et fièvre, puis signes respiratoires/GI/peau/yeux et progression neurologique.
"CDV is shed in respiratory secretions for up to 90 days after infection. Initial replication of CDV is in lymphoid tissue, and viral destruction of lymphocytes results in lymphopenia and pyrexia."
Causes et facteurs de risque
Aérosol et liquides corporels. Chiots non vaccinés 6–12 semaines et furets à très haut risque; carnivores sauvages comme réservoir. Pas de prédisposition raciale.
Signes à surveiller
- Myoclonies (cardinal) au repos/sommeil
- Fièvre, dépression, écoulement mucopurulent, toux, vomissements/diarrhée, pustules, hyperkératose nez/coussinets, convulsions, ataxie (fréquents)
- Uvéite, KCS, avortement, encéphalite du vieux chien (occasionnels)

« Hard pad »: coussinets et nez épaissis et craquelés.
Diagnostic
Clinique + NFS (lymphopénie), radiographies thoraciques, cytologie conjonctivale, RT-PCR (référence), IHC, parfois LCR. Interpréter la PCR avec l'historique vaccinal.

La radio évalue la pneumonie secondaire.
Traitement
Pas d'antiviral curatif. Support: fluides IV, antibiotiques contre surinfections, thérapie respiratoire, antiémétiques, anticonvulsivants. Glucocorticoids sélectifs selon la phase. Isolement strict jusqu'à 90 jours d'excrétion.
Pronostic
Souvent mauvais avec signes SNC; furets presque toujours fatals une fois cliniques. Survivants peuvent garder myoclonies, KCS ou hypoplasie de l'émail.

Prévention
Vaccin core chiots 6–18 semaines, rappels; vaccin spécifique furet; quarantaine; désinfection.
Quand appeler le vétérinaire
Urgence chez animal non vacciné avec convulsions/myoclonies, dyspnée, écoulement purulent, collapsus, vomissements/diarrhée graves, cécité brutale.
