TL;DR. Sattelthrombus: plötzlicher Notfall, bei dem ein Gerinnsel den Blutfluss zu den Hinterbeinen blockiert; extreme Schmerzen und Lähmung.

Der Sattelthrombus sitzt an der Aortentrifurkation und sperrt die Hintergliedmaßen ab.

Was ist das?

Embolus aus dem Herzen (oft linker Vorhof) bleibt an der Aortengabel stecken. Ischämie → Rhabdomyolyse, Nervenschaden, Paresis/Paralyse.

Ursachen und Risikofaktoren

Katze: meist HCM mit vergrößertem Vorhof. Hund: Hyperkoagulabilität (Cushing, proteinverlierende Nierenerkrankung, IMHA, Tumor, Sepsis, Pankreatitis u. a.).

Anzeichen

  • Starke Schmerzen, Hinterhandparese/-paralyse, fehlende Femoralpulse (kardinal)
  • Kalte, blasse/zyanotische Pfoten, Tachypnoe, Hypothermie (häufig)
  • Muskelkontraktur, Herzgeräusch/Arrhythmie (gelegentlich)

Diagnose

Klinik der „5 P“. Echo, Abdominalsono/Doppler, Thorax-Röntgen vor Diuretika, CK/AST, ggf. TEG.

Behandlung

Schmerz (Opioide). Antikoagulation: Heparin, LMWH; langfristig Clopidogrel, ggf. Aspirin. Sauerstoff, vorsichtige Flüssigkeit. Reperfusionsverletzung (Hyperkaliämie) überwachen.

Prognose

Schlecht: hohe Sterblichkeit/Euthanasie in der Akutphase; Rezidive häufig.

Vorbeugung

Früherkennung kardialer Erkrankung; Clopidogrel bei relevantem LA-Enlargement. Grundleiden beim Hund behandeln.
