Feline Infectious Peritonitis (FIP) is an uncommon but serious viral disease in cats caused by a mutation of the common feline coronavirus. It occurs in "wet" (effusive) and "dry" (noneffusive) forms, causing progressive inflammation throughout the body. While historically considered fatal, understanding the diagnostic process and supportive care options is crucial for managing affected cats.

Vad är det?
Ovanlig systemisk sjukdom. FCoV är vanligt och oftast milt. Hos få katter mutation till FIPV i makrofager med kraftig inflammation.
- Effusiv («våt»): vaskulit och proteinrik gul vätska.
- Icke-effusiv («torr»): pyogranulom i njure, lever, lymfknutor, öga, CNS.
Orsaker och riskfaktorer
Mutation i individen; FIP smittar sällan katt-katt (FCoV via avföring). Risk: <2 år eller mycket gamla; stress; raser British Shorthair, Devon Rex, Abyssinian.
Tecken att bevaka
- Vanliga: viktnedgång, fluktuerande feber, anorexi, letargi
- Ibland: uppsvälld buk, dyspné, ikterus, uveit, ataxi, nystagmus/kramper, kräkning/diarré
Anterior uveit är ett tillfälligt tecken på torr FIP.
Hur veterinärer diagnostiserar
Svår diagnos. Blod: höga globuliner, lågt albumin, lågt A:G. Våt: Rivalta, RT-PCR, immunocytokemi. Torr: bilddiagnostik + biopsi. Antikroppar = bara exponering. Guldstandard: histopatologi med immunfärgning.
Behandling
Historiskt nästan alltid dödlig utan moderna antivirala. Palliativt glukokortikoider; interferon omega; stöd. GS-441524-tillgänglighet varierar regionalt.
Prognos
Dålig utan avancerad antiviral behandling; våt snabbare. Bättre där moderna antivirala finns.
Prevention
Hygien i flerkattshem, mindre stress. Isolering p.g.a. «FIP-smitta» behövs inte enligt källor.
När kontakta veterinären
Akut: andnöd, kramper, anorexi >24-48 h, extrem svaghet, tydlig ikterus.
