Feline Infectious Peritonitis (FIP) is an uncommon but serious viral disease in cats caused by a mutation of the common feline coronavirus. It occurs in "wet" (effusive) and "dry" (noneffusive) forms, causing progressive inflammation throughout the body. While historically considered fatal, understanding the diagnostic process and supportive care options is crucial for managing affected cats.

La PIF débute souvent par des signes vagues : léthargie, baisse d'appétit, amaigrissement.

Qu'est-ce que c'est ?

Maladie systémique peu fréquente. Le FCoV est très répandu (surtout en collectivité) et souvent bénin. Chez un faible pourcentage de chats, mutation en FIPV qui infecte les macrophages et provoque une inflammation destructive.

  • PIF effusive (« humide ») : vascularite et liquide jaunâtre riche en protéines (abdomen/thorax).
  • PIF non effusive (« sèche ») : pyogranulomes (reins, foie, nœuds, yeux, SNC).

Causes et facteurs de risque

Mutation intra-hôte ; la PIF ne se propage pas facilement chat-à-chat (le FCoV oui, fèces). Risques : <2 ans ou très âgés ; stress ; races suspectes British Shorthair, Devon Rex, Abyssinian.

Signes à surveiller

  • Fréquents : amaigrissement, fièvre fluctuante, anorexie, léthargie
  • Occasionnels : ballonnement abdominal, dyspnée, ictère, uvéite, ataxie, nystagmus/convulsions, vomissements/diarrhée

Comment les vétérinaires diagnostiquent

Diagnostic difficile. Biochimie : hyperglobulinémie, hypoalbuminémie, ratio A:G bas. Forme humide : liquide paille épais ; Rivalta, RT-PCR, immunocytochimie. Forme sèche : imagerie + biopsie. Sérologie = exposition seulement. Référence : histopathologie + immunostaining.

Options de traitement

Historiquement presque toujours fatale sans antiviraux modernes. Glucocorticoïdes en première intention palliative. Interféron oméga et soins de soutien (propentofylline, stanozolol, etc.). Disponibilité des antiviraux type GS-441524 variable selon les pays.

Pronostic

Réservé sans antiviral avancé ; forme humide plus rapide. Pronostic bien meilleur là où les antiviraux modernes sont accessibles.

Prévention

Hygiène multi-chats, litières propres, moins de stress. Isolation pour « contagion PIF » non requise selon les références critiques.

Quand appeler le vétérinaire

Urgence : dyspnée, convulsions, anorexie >24-48 h, prostration, ictère marqué.
