Feline Infectious Peritonitis

Feline Infectious Peritonitis

Auch bekannt als: FIP

Tierärztlich geprüftCatDringlichkeit: HighHäufigkeit: Uncommon

Kurz gesagt

Feline Infectious Peritonitis (FIP) is an uncommon but serious viral disease in cats caused by a mutation of the common feline coronavirus. It occurs in "wet" (effusive) and "dry" (noneffusive) forms, causing progressive inflammation throughout the body. While historically considered fatal, understanding the diagnostic process and supportive care options is crucial for managing affected cats.

A domestic cat sitting quietly, looking slightly lethargic.
FIP often begins with vague signs such as lethargy, decreased appetite, and weight loss.

TL;DR.

FCoV ist weit verbreitet (v. a. Mehrkatzenhaltung) und meist harmlos. Bei wenigen Katzen Mutation zu FIPV, Invasion von Makrophagen, schwere Entzündung.

  • Effusiv («nass»): Vaskulitis, proteinreiches gelbes Fluid in Bauch/Brust.
  • Nichteffusiv («trocken»): Pyogranulome in Niere, Leber, Lymphknoten, Auge, ZNS.

Ursachen und Risikofaktoren

Mutation im Individuum; FIP selbst kaum katzenansteckend (FCoV ja, fäkal-oral). Risiko: <2 Jahre oder sehr alt; Stress; Rassen British Shorthair, Devon Rex, Abyssinian.

Anzeichen

  • Häufig: Gewichtsverlust, fluktuierendes Fieber, Anorexie, Lethargie
  • Gelegentlich: Bauchauftreibung, Dyspnoe, Ikterus, Uveitis, Ataxie, Nystagmus/Krämpfe, Erbrechen/Durchfall

A close-up of a cat's eye showing anterior uveitis, with cloudiness and inflammation.
Anteriore Uveitis ist ein gelegentliches Zeichen der trockenen FIP.

Diagnose

Kein einzelner nicht-invasiver Goldtest. Blut: hohe Globuline, niedriges Albumin, niedriges A:G (oft <0,4-0,8). Nass: Rivalta, RT-PCR, Immunzytochemie. Trocken: Bildgebung + Biopsie. Antikörpertiter = nur Exposition. Goldstandard: Histopathologie mit Immunfärbung.

Behandlung

Historisch fast immer tödlich ohne moderne Antivirale. Palliativ Glukokortikoide; Interferon Omega; Supportiva. GS-441524 u. ä. verändern die Prognose, Verfügbarkeit regional unterschiedlich.

Prognose

Ohne fortgeschrittene antivirale Therapie ungünstig; nass schneller. Mit modernen Antiviralen deutlich besser, wo legal verfügbar.

Vorbeugung

FCoV-Hygiene in Mehrkatzenhaushalten, Stress senken. Isolierung wegen «FIP-Ansteckung» laut Referenz nicht nötig.

Wann den Tierarzt rufen

Notfall: Atemnot, Krampfanfälle, Anorexie >24-48 h, extreme Schwäche, starker Ikterus.
