O que é?

Os átrios fibrilam sem contração eficaz; o nó AV deixa passar demasiados impulsos; ventrículos batem rápido e irregular. O débito cai.

"This common arrhythmia is characterized by rapid and chaotic electrical activation within the atria. There are no P waves on the ECG because there is no uniform atrial depolarization wave. Rather, the baseline usually shows irregular undulations (fibrillation waves). Lack of organized electrical activity prevents effective atrial contraction."

Causas e fatores de risco

Geralmente secundária a doença estrutural (DCM, válvulas, congênitos, HCM). Lone AF em raças gigantes. Predispostos: Doberman Pinscher, Great Dane, Irish Wolfhound.

Sinais

  • Cardinal: ritmo totalmente irregular
  • Frequentes: déficits de pulso, fraqueza, baixa tolerância ao exercício
  • Ocasionais: tosse, dispneia, síncope, ascite

Monitore a frequência respiratória em repouso em casa.

Diagnóstico

Auscultação + ECG (padrão-ouro). Radiografias, eco, Holter.

Tratamento

Controle da frequência, não cardioversão habitual.

"In patients with underlying cardiac dysfunction, cardiac output tends to decrease considerably when AF develops. Long-lasting conversion to sinus rhythm is rare in the face of marked underlying cardiac disease, even after successful electrical cardioversion. Therefore treatment in most cases is directed at reducing the ventricular response rate by slowing AV conduction..."

Primeira linha: Diltiazem; Digoxin.

Segunda linha: amiodarone, Sotalol, Lidocaine.

Prognóstico

Reservado com doença estrutural; melhor em lone AF de baixa frequência.

Prevenção

Detecção precoce em raças de risco.

Quando chamar o veterinário

Emergência: dispneia; FR em repouso >30–40/min; colapso/síncope; ascite; mucosas pálidas/azuladas.
