Atrial fibrillation is a common, serious heart arrhythmia in dogs and cats characterized by rapid, chaotic electrical signals in the upper heart chambers. While it often stems from underlying heart disease, some giant breed dogs experience "lone" atrial fibrillation with a more favorable outlook.

Qu'est-ce que c'est ?

Les oreillettes fibrillent sans contraction efficace; le nœud AV laisse passer trop d'impulsions; les ventricules battent vite et irrégulièrement. Le débit chute.

"This common arrhythmia is characterized by rapid and chaotic electrical activation within the atria. There are no P waves on the ECG because there is no uniform atrial depolarization wave. Rather, the baseline usually shows irregular undulations (fibrillation waves). Lack of organized electrical activity prevents effective atrial contraction."

Causes et facteurs de risque

Souvent secondaire à une maladie structurelle (MCD, valves, congénital, HCM). FA « lone » chez races géantes. Prédisposés: Doberman Pinscher, Great Dane, Irish Wolfhound.

Signes

  • Cardinal: rythme totalement irrégulier
  • Fréquents: déficits de pouls, faiblesse, mauvaise tolérance à l'effort
  • Occasionnels: toux, dyspnée, syncope, ascite

Surveillez la fréquence respiratoire au repos à la maison.

Diagnostic

Auscultation + ECG (référence). Radiographies, écho, Holter.

Traitement

Contrôle de la fréquence, pas de cardioversion habituelle.

"In patients with underlying cardiac dysfunction, cardiac output tends to decrease considerably when AF develops. Long-lasting conversion to sinus rhythm is rare in the face of marked underlying cardiac disease, even after successful electrical cardioversion. Therefore treatment in most cases is directed at reducing the ventricular response rate by slowing AV conduction..."

Première intention: Diltiazem; Digoxin.

Deuxième intention: amiodarone, Sotalol, Lidocaine.

Pronostic

Réservé avec maladie structurelle; meilleur en lone AF à basse fréquence.

Prévention

Dépistage précoce chez races à risque.

Quand appeler le vétérinaire

Urgence: dyspnée; FR au repos >30–40/min; collapsus/syncope; ascite; muqueuses pâles/bleutées.
