¿Qué es?

Las aurículas fibrilan sin contracción eficaz; el nódulo AV deja pasar demasiados impulsos y los ventrículos laten rápido e irregular. Cae el gasto cardiaco.

"This common arrhythmia is characterized by rapid and chaotic electrical activation within the atria. There are no P waves on the ECG because there is no uniform atrial depolarization wave. Rather, the baseline usually shows irregular undulations (fibrillation waves). Lack of organized electrical activity prevents effective atrial contraction."

Causas y factores de riesgo

Suele ser secundaria a enfermedad estructural (MCD, válvulas, defectos congénitos, HCM). AF «lone» en razas gigantes. Predispuestos: Doberman Pinscher, Great Dane, Irish Wolfhound.

Signos

  • Cardinal: ritmo totalmente irregular
  • Frecuentes: déficits de pulso, debilidad, baja tolerancia al ejercicio
  • Ocasionales: tos, disnea, síncope, ascitis

Diagnóstico

Auscultación + ECG (estándar de oro). Radiografías, eco, Holter.

Tratamiento

Control de frecuencia, no cardioversión habitual.

"In patients with underlying cardiac dysfunction, cardiac output tends to decrease considerably when AF develops. Long-lasting conversion to sinus rhythm is rare in the face of marked underlying cardiac disease, even after successful electrical cardioversion. Therefore treatment in most cases is directed at reducing the ventricular response rate by slowing AV conduction..."

Primera línea: Diltiazem; Digoxin.

Segunda línea: amiodarone, Sotalol, Lidocaine.

Pronóstico

Reservado con enfermedad estructural; mejor en lone AF de baja frecuencia.

Prevención

Detección precoz en razas de riesgo.

Cuándo llamar al veterinario

Emergencia: disnea; FR en reposo >30–40/min; colapso/síncope; ascitis; mucosas pálidas/azuladas.
