Hvad er det?
Knoglemarven danner erytrocytter, leukocytter og trombocytter. Ved aplastisk anæmi erstattes aktivt væv af fedt/fibrose/nekrose og pancytopeni opstår.
"suggests a defect either in the marrow precursor cells themselves or in the supportive microenvironment that the cells depend on, including cytokines, hormones, and growth factors. The remaining marrow (≥75%) is composed of fatty tissue, fibrosis, necrosis, or gelatinous transformation (serous atrophy of fat)."
Årsager og risikofaktorer
Infektioner (FeLV kat, Ehrlichia canis hund), lægemidler (kemoterapi, visse antibiotika, østrogener, NSAID som fenylbutazon), toksiner/stråling, immunmedieret; ofte idiopatisk. Ingen kendte raceprædispositioner.
Tegn
- Pancytopeni, anæmi (kardinale): svaghed, blege slimhinder.
- Trombocytopeni, neutropeni (almindelige)
- Feber, spontan blødning, petekkier (lejlighedsvise)
Blege slimhinder og petekkier tyder på mangel på erytrocytter og trombocytter.
Diagnostik
CBC (ikke-regenerativ anæmi + pancytopeni), FeLV kat, Ehrlichia hund. Guldstandard: knoglemarvsbiopsi med histopatologi.
Biopsien viser erstatning af bloddannende celler med fedt.
Behandling
Glukokortikoider ved immunmedieret form; transfusioner, profylaktiske antibiotika, seponering af marvtoksiske lægemidler.
Prognose
Dårlig ved FeLV; idiopatisk/immun forsigtigt håbefuld med aggressiv support.
Forebyggelse
Indendørs katte + FeLV-vaccine; helårs flåtprofylakse hos hund; aldrig humanmedicin uden dyrlæge.
Hvornår man skal ringe til dyrlægen
Akut ved meget blege slimhinder, kollaps, blødning, petekkier, åndenød eller feber med letargi.
