Là gì?
Tủy tạo hồng cầu, bạch cầu và tiểu cầu. Trong thiếu máu bất sản mô hoạt động bị thay bằng mỡ/xơ/hoại tử và xuất hiện pancytopenia.
"suggests a defect either in the marrow precursor cells themselves or in the supportive microenvironment that the cells depend on, including cytokines, hormones, and growth factors. The remaining marrow (≥75%) is composed of fatty tissue, fibrosis, necrosis, or gelatinous transformation (serous atrophy of fat)."
Nguyên nhân và yếu tố nguy cơ
Nhiễm trùng (FeLV mèo, Ehrlichia canis chó), thuốc (hóa trị, kháng sinh nhất định, estrogen, NSAID như phenylbutazone), độc tố/xạ, miễn dịch; thường vô căn. Không có khuynh hướng giống đã biết.
Dấu hiệu
- Pancytopenia, thiếu máu (cốt lõi): yếu, niêm mạc nhợt.
- Giảm tiểu cầu, giảm bạch cầu trung tính (thường)
- Sốt, chảy máu tự phát, petechiae (đôi khi)
Nướu nhợt và petechiae cho thấy thiếu hồng cầu và tiểu cầu.
Chẩn đoán
CBC (thiếu máu không tái tạo + pancytopenia), FeLV mèo, Ehrlichia chó. Tiêu chuẩn vàng: sinh thiết tủy với mô bệnh học.
Sinh thiết cho thấy thay tế bào tạo máu bằng mỡ.
Điều trị
Glucocorticoid nếu miễn dịch; truyền máu, kháng sinh dự phòng, ngừng thuốc độc tủy.
Tiên lượng
Xấu với FeLV; vô căn/miễn dịch thận trọng có hy vọng với hỗ trợ mạnh.
Phòng ngừa
Mèo trong nhà + vắc-xin FeLV; phòng ve quanh năm cho chó; không thuốc người khi chưa có thú y.
Khi nào gọi bác sĩ thú y
Cấp cứu nếu niêm mạc rất nhợt, xẹp, chảy máu, petechiae, khó thở hoặc sốt kèm lờ đờ.
