Letargie en zwakte zijn vroege tekenen van ernstige bloedstoornissen.
Wat is het?
Beenmerg produceert erytrocyten, leukocyten en trombocyten. Bij aplastische anemie wordt actief merg vervangen door vet/fibrose/necrose en ontstaat pancytopenie.
"suggests a defect either in the marrow precursor cells themselves or in the supportive microenvironment that the cells depend on, including cytokines, hormones, and growth factors. The remaining marrow (≥75%) is composed of fatty tissue, fibrosis, necrosis, or gelatinous transformation (serous atrophy of fat)."
Oorzaken en risicofactoren
Infecties (FeLV kat, Ehrlichia canis hond), medicijnen (chemo, bepaalde antibiotica, oestrogenen, NSAID zoals fenylbutazon), toxinen/straling, immuungemedieerd; vaak idiopathisch. Geen bekende raspredisposities.
Signalen
- Pancytopenie, anemie (cardinaal): zwakte, bleke slijmvliezen.
- Trombocytopenie, neutropenie (vaak)
- Koorts, spontane bloeding, petechiën (soms)
Bleke slijmvliezen en petechiën wijzen op tekort aan erytrocyten en trombocyten.
Diagnose
CBC (niet-regeneratieve anemie + pancytopenie), FeLV kat, Ehrlichia hond. Goudstandaard: beenmergbiopsie met histopathologie.
Biopsie toont vervanging van bloedvormende cellen door vet.
Behandeling
Glucocorticoïden bij immuungemedieerd; transfusies, profylactische antibiotica, stop myelotoxische middelen.
Prognose
Slecht bij FeLV; idiopathisch/immuun voorzichtig hoopvol met agressieve support.
Preventie
Katten indoor + FeLV-vaccin; jaarronde tekenpreventie hond; nooit humaan geneesmiddelen zonder dierenarts.
Wanneer de dierenarts bellen
Spoed bij zeer bleke slijmvliezen, collaps, bloeding, petechiën, ademnood of koorts met letargie.
